• Serve as a resource in assisting field staff with problem solving to provide the most appropriate care management for our patients
• Maintain a strong working knowledge of Medicare, Medicaid, and private insurance guidelines related to admission and service criteria and its proper clinical documentation
• Assist with patient visits as needed
• Complete documentation and chart reviews to identify variances in standardized care and care processes to identify areas for improved performance and prepare for all regulatory audits
• Work collaboratively with all team members and external stakeholders to coordinate care and achieve patient and program goals
• Ensure appropriate resource utilization relevant to the financial, regulatory, and clinical aspects of care
• Act as role model or preceptor for new team members as needed

• Associates degree in nursing, physical, occupation, or speech therapy required
• Current and unrestricted RN, PT, OT, or ST clinical license in state of practice
• 2 years home health clinical experience or health care management experience required
• Branch operation experience preferred
• Valid and unrestricted driver's license required
