Location: CSM Fire House - 1444 7th Street, Santa Monica, CA 90401

Setting: 100% Onsite

Schedule: Monday - Friday, 8:00am - 4:30pm

Status: Full-time/Non-Exempt/Non-Management

Job Description Summary:

With the support of and in partnership with the Santa Monica C3 Program Manager and Westside Outreach Leadership, the Senior Clinical Case Manager performs a variety of tasks to assist the most vulnerable and chronically homeless individuals in high density areas of Santa Monica. The Senior Clinical Case Manager is responsible for identifying, engaging and providing support to homeless clients, including providing referrals, linkages, and coordinating care. The Senior Clinical Case Manager provides information and support for co-workers and assists in training and mentoring by linking to a variety of resources and services as needed. The Senior Clinical Case Manager will provide risk assessments and high-level clinical interventions and assessments, assisting clients and the team in attaining wellness and safety.

Job Description Essential Duties and Responsibilities:



  • Provide field-based services, meeting currently homeless clients where they are in order to eliminate barriers to services.
  • Assess mental health service needs of clients.
  • Engage clients in developing mutually agreed-upon goals documented in each client's confidential service plan.
  • Provide risk assessment and clinical evaluations for client's who are potentially gravely disabled, or at risk of harm to self or others and in need of hospitalization.
  • Ability to initiate or be trained to initiate psychiatric holds by becoming LPS designated.
  • When appropriate, participate in navigation the referral process to link an individual to higher levels of care.
  • Link clients with appropriate medical, mental health, substance abuse, and vocational resources.
  • Assist clients in obtaining all benefits to which they are entitled, including applying for and advocating for SSI benefits.
  • Provide opportunities for clients to build a supportive peer community.
  • Build relationships with County Mental Health Department and other resources to increase collaboration and responsiveness to clients with severe mental illness and addiction.
  • Complete all required documentation in a timely manner.
  • Collaborate and communicate effectively with police officers and local hospital staff to reduce police contact and ED visits by targeted population.
  • Maintain an effective working relationship with Santa Monica community partners (including police, firefighters, city staff, etc.) and maintain and build professional rapport.
  • Attend all case conference and community/funder meetings to ensure collaboration and continuity of care for the clients we serve.
  • Work effectively on an interdisciplinary team and collaborate with Program Manager to ensure high level of clinical oversight.
  • Other duties as assigned.



Job Description Qualifications:



  • Experienced Mental Health (Licensed or near-licensed) professional (LCSW, LMFT, PhD, PsyD) with at least 2 years of experience working with individuals with serious mental illness, substance abuse, and physical health problem.
  • Capacity and willingness to get LPS designated for initiating psychiatric holds
  • Able to work collaboratively as part of an interdisciplinary team
  • Valid CA Driver's license, reliable car, auto insurance, and an acceptable driving record



Job Description Work Environment:



  • Field (may need to travel) and indoor office environment.
  • Regularly required to sit, stand, bend; occasionally lift up to 35lbs.
  • Regularly walk or drive to different local sites.
  • Will necessitate working in busy and loud environments
  • Will be exposed to elements like cold, heat, dust, noise, and odor.
  • May need to bend, stoop, twist, and sit throughout the day.
