Starting Pay: $22.98/hr

The Community Engagement Facilitator provides support, assistance, and guidance to individuals with disabilities that will enable the individual to develop skills and enhance their ability to engage with their community. This position is responsible for providing day-to-day assistance and support to consumers by monitoring and supervising consumers in the community, assisting with task and skill training, implementing ISP outcomes, and providing personal care essential support as needed.

The Community Engagement completes quarterly reports and writes Individualized Service Plans (ISP). This position is expected to work cooperatively with their supervisor, and collaboratively with fellow employees, and consumers of service.



  • Coordinate activities with consumers, pair individuals with similar interests, and provide Community-Based community engagement activities to a maximum of 3 individuals per session.
  • Facilitates activity exploration with consumers via telehealth/virtual meetings in order to determine the schedule each week.
  • Provide direct support to consumers in accordance with written ISP outcomes.
  • Monitor and document daily individual consumer progress toward written ISP outcomes.
  • Maintains and follows a daily schedule of pre-planned community activities.
  • Provides feedback to assist in the development of consumer outcomes.
  • Inform the Director of the consumer's achievements, incidents, progress, or problems as needed.
  • Assists in the creation and facilitation of specialized training and community integration.
  • Provides one-on-one specialized assistance to consumers with identified needs for additional support.
  • Respects Human Rights and serves as an advocate for consumers.
  • Writes Individualized Service Plans (ISP) in accordance with Didlake policies and procedures.
  • Completes Quarterly reports and daily/weekly documentation on individual progress.
  • Educates consumers regarding services/employment options that reflect consumer choice as a first priority.
  • Encourages and educates consumers on the benefits of choices that maximize community integration.
  • Deliver Day Support services that are in full compliance with Home and Community-Based Services (HCBS) Settings Regulations;
  • In a setting that is integrated into and supports full access to the greater community.
  • Is selected by the individual from a variety of settings options.
  • Ensures individual rights of privacy, dignity, respect, and freedom from coercion and restraint.
  • Optimizes autonomy and independence in making life choices.
  • Facilitates choice regarding services and who provides them.
  • Requires that the person-centered planning process is directed by the individual or individual's chosen representative.
  • Outlines the minimum requirements for person-centered service plans with individually identified goals and preferences.
  • Informs and respects consumer choice with dignity of risk in mind.
  • Follow safety protocols.
  • Clean and disinfect vehicles to assist with reduction of illnesses.
