Be the Best After Hours Hospice LVN/LPN You Can Be

If you meet these qualifications, we would love to meet you:

  • Must hold a current Licensed Practical Nurse (LPN) license in the practicing state(s); associate degree preferred. CPR certification is required for direct patient care in Georgia and preferred elsewhere

  • Requires at least one year of nursing experience in hospice, home health, or a specific patient population (e.g., geriatrics, pediatrics, med-surg)

  • Strong written, verbal, and organizational skills, along with proficiency in Microsoft Office and EMR systems

  • Must have a valid driver's license, reliable vehicle, and meet minimum state-required auto insurance

  • Able to work as part of an interdisciplinary team with knowledge of relevant state, agency, and federal hospice policies and protocols

  • Physical demands include frequent lifting (up to 50 lbs), constant assistance with patients (up to 200 lbs), frequent walking, stooping, driving, grasping, and computer use, with occasional exposure to environmental conditions such as temperature changes, noise, chemicals, and uneven walking surfaces

Responsibilities:

  • Assess and gather information about the physical, emotional, social, and spiritual needs of patients and families, in coordination with the interdisciplinary care team
  • Communicate regularly with the Hospice RN, care team, leadership, and external agencies to ensure seamless, high-quality care and timely issue resolution
  • Maintain accurate, up-to-date documentation that reflects the patient's evolving condition, care goals, and interventions
  • Participate in care services such as Continuous Care, Respite, On-Call rotation, and Information/Consent visits, as assigned
  • Provide patient and caregiver education tailored to their needs, helping them understand and follow the care plan with confidence
  • Attend team meetings, contribute to orientation and training, and support other tasks or projects as needed within the scope of licensure
