Job Description Summary:

Manages care coordination team staff and social work activities including patient care, case review, care plan creation and evaluation, performance feedback and professional development. Practices professional social work and promotes advocacy in accordance with the NASW Code of Ethics for Social Work and maintains a current Ohio Social Work license.

Job Description:

Essential Functions:

  • Performs all functions of the MSW Care Coordinator through management of their own caseload.
  • Oversees care coordination team staff in documentation, patient care, staffing, and clinical competencies. Serves as a clinical expert for policies, procedures, documentation, and skills.
  • Assists in the training and development of new and existing staff members, including performance appraisal.
  • Assists in supervising clinical operations. Explains why clinical and financial management are important to the organization.
  • Engages in the "workplace community" by staying aware of changes, participating in data collection or auditing, understanding Quality Improvement, and representing staff at relevant meetings and then reporting important information and updates back to staff.
  • Maintains safety and quality as priorities, providing auditing and education for safety and quality initiatives.
  • Ensures regulatory compliance and emergency response preparedness.

Education Requirement:

MSW

Licensure Requirement:

  • LISW, LISW-S preferred and required to be obtained within 1 year of hire.
  • Valid Ohio driver's license and proof of auto insurance as required by hospital policy and position-specific requirements. Must pass motor vehicle background inspection, insurance eligibility, driving qualifications, and training set forth by Nationwide Children's Hospital and maintain qualification of insurance guidelines.

Experience:

3 years' experience in pediatric clinical care or case management.
