Psychiatrist, Bridge to Home (BTH)

Job Summary:

Bridge to Home (BTH) is a new innovative program developed by NYC Health + Hospitals that will provide treatment, social and professional skill building, and temporary housing to patients with serious mental illness (SMI) who are ready for discharge from the hospital but do not have immediate access tostable housing.

The psychiatrist, in partnership with the Site Director, holds primary clinical responsibility for psychiatric care and clinical oversight to nursing at the BTH residence. This includes assessment, treatment planning, psychopharmacology, and team consultation, with a focus on recovery-oriented and trauma-informed care.

General responsibilities:

Provide psychiatric and medical care to all assigned patients in collaboration with nursing staff, other disciplines, and consultants from other services when applicable.

Adhere to appropriate clinical documentation to ensure compliance with current standards of clinical practice and adherence to all regulatory and accreditation standards.

Maintain effective communication with patients, patients' families when applicable, and other disciplines.

Specific responsibilities:

Provide clinical leadership alongside the Site Director in the areas of psychiatric assessment, diagnosis, treatment planning and comprehensive risk assessments.

Conduct initial and follow-up psychiatric evaluations, including mental status exams and diagnostic formulation in accordance with DSM-5 criteria.

Prescribe and manage psychotropic medications; monitor effectiveness and side effects.

Provide psychoeducation to patients and, when appropriate, their families, on diagnosis, treatment options, and medication.

Provide on-site support and clinical support for nursing.

Collaborate with the multidisciplinary team to ensure holistic, person-centered care.

Consult with staff and external providers (e.g., inpatient units, primary care, housing programs) to support continuity of care.

Participate in case conferences, treatment team meetings, and clinical supervision.

Provide crisis assessment and intervention when needed.

Contribute to discharge planning and housing transitions.

Document care in accordance with Part 599 OMH Regulations and NYC H+H policies.

Provide occasional after-hours or weekend consultation by phone, if needed, to support onsite staff.

Maintain compliance with HIPAA, licensing, and organizational standards.

Report to the BTH Site Director for day-to-day clinical operations and to the Medical Director of Ambulatory Behavioral Health for professional practice and supervision.

Other functions as directed by the Bellevue Hospital's Director of Ambulatory Psychiatry or Chief of Adult Psychiatry.

Compliance:

Be familiar with all departmental policies and protocols.

Complete all required activities related to maintaining credentialing and other mandates at both Bellevue and NYU.

Complete yearly flu vaccination and fit test requirements and documentation.

Documentation:

Ensure that all documentation in the EMR is accurate and complete.

Complete legal documents on patients if applicable.

Licensed to practice medicine in the state of New York with American Board of Psychiatry Certification.

Preferred Qualifications

Completion of a Public Psychiatry Fellowship.

Experience working with individuals with SMI, preferably in an acute, residential, community-based or integrated care setting.

Operational and clinical management experience in a community mental health setting.
