JOB SUMMARY:

Here at Professional Services Group, we are a community-based social services organization looking for a Rehabilitation and Support Specialist to join our CCS Ancillary Services program in Milwaukee County. The program uses goal-oriented interventions to support both youth and adult clients referred through the Comprehensive Community Services (CCS). As the Rehabilitation and Support Specialist, you will provide individual skill development and rehabilitation services. Services may include individual meetings and structured rehabilitation activities that build a variety of different skills such as community integration, housing, money management, social skills, or crisis intervention.

Position will be part-time to start, with the opportunity to grow into a full-time position. This is a great opportunity for experienced professionals seeking part-time hours and recent graduates looking to transition into the human services/mental health field.

KEY RESPONSIBILITIES OF THE REHABILITATION AND SUPPORT SPECIALIST:



  • Analyze the client's capabilities, strengths, and problems to determine the services required to meet their unique needs.
  • Implement, evaluate, and review Recovery Plan of Care goals in collaboration with team members, other providers, and natural supports.
  • Exercise judgment to intervene as an advocate for the consumer in the community and as part of their recovery team.
  • Attend recovery team meetings as needed.
  • Formulate units, monthly reports, and discharge summaries.
  • Organize and facilitate individual meetings and structured rehabilitation activities that assist consumers in developing pro-social skills and connections.
  • Safely transport clients as needed.


LOCATION: PSG West Allis office. Local travel may be required to work within client homes or other community spaces, with mileage reimbursement. Flexibility is available for some hybrid/work-from-home hours when on-site meetings are not scheduled.

SCHEDULE: Part-time leading to full-time. Hours are varied, typically during business hours Monday-Friday between 9:00am and 6:00pm.

SALARY: $21.00-23.00 per hour

REQUIREMENTS:


  • Must have a Bachelor's Degree in Social Work, Psychology, Criminal Justice, or related.
  • Must have reliable transportation on a daily basis, a valid driver's license, and automobile insurance.
  • Previous experience working with at-risk populations is preferred.
