Job Summary and Responsibilities

Job Summary

Join Acentra Health as a Clinical Evaluator (PASRR) in Scottsbluff, Nebraska, and make a meaningful impact in your community. This hybrid role includes conducting in-person behavioral health assessments across nursing homes, hospitals, and community-based settings in Scottsbluff, and nearby areas.

As a Clinical Evaluator, you will conduct face-to-face PASRR Level II assessments to determine appropriate placements and next levels of care for individuals with complex needs. Your work will focus on clinical evaluations for individuals with Serious Mental Illness (SMI), Intellectual Disabilities (ID), or Related Conditions (RC) across nursing homes, hospitals, and community-based settings. These evaluations are essential for determining the appropriate next level of care and contribute to improved patient well-being and health outcomes.

This PRN hybrid role offers flexibility and purpose combining remote work from your home office with local travel. Acentra Health reimburses all travel-related expenses, including mileage and parking, and provides all required technology and equipment, including a laptop for fieldwork.

Job Responsibilities

  • Responsible for conducting PASRR (Preadmission Screening and Resident Review) assessments and evaluations, and for developing informed level-of-care recommendations across diverse settings-including nursing homes, hospitals, and community environments to effectively understand and address the unique needs of each individuals served.
  • Adhere to the state's best standards of mental health practices, intellectual and developmental disabilities, geriatrics, and other conditions requiring nursing care services and supports.
  • Ensure compliance with contractual and regulatory standards while developing level of care recommendations
  • Utilize independent judgment, deep clinical expertise, exceptional communication skills, and problem-solving abilities to navigate complex situations, ensuring the best outcomes for members, providers, and clients.
  • Foster effective collaboration with a diverse range of healthcare professionals, patients, families, and support networks while monitoring personal metrics to ensure compliance with turnaround time requirements.
  • Ensure compliance with established workflows, including re-evaluation of records based on psychiatric input.
  • Stay informed on current literature, regulations, and research pertaining to long-term care, mental illness, medical conditions, and geriatrics.
  • Leverage technology by utilizing the state's electronic system and our proprietary platforms to efficiently manage and track all assessment activities.
  • Execute tasks with strong attention to detail, ensuring adherence to established workflows to maintain high standards of quality.

Qualifications

Required Qualifications, Skills, Knowledge and Experience

  • Must possess an active, unrestricted license in the State of Nebraska in one of the following:Licensed Clinical Social Worker (LCSW), Licensed Master Social Worker (LMSW), Licensed Marriage and Family Therapist (LMFT), Licensed Independent Clinical Social Worker (LICSW), Licensed Clinical Professional Counselor (LCPC), Licensed Clinical Psychologist (LCP), Registered Nurse (RN), or another qualified Licensed Mental Health Professional (LMHP).
  • Graduation from an accredited college or university with a degree relevant to the field, and minimum of 2 years of experience assessing and diagnosing individuals with mental or psychiatric disorders-including expertise in complex differential diagnoses involving substance use, medical conditions, cognitive impairments, and dementia.
  • Minimum 1 year of experience working with individuals with intellectual disabilities or related conditions.
  • Proven ability to assess and analyze psychological, emotional, behavioral, and physical functioning to support accurate and thorough clinical evaluations.
  • Knowledge of Level of Care standards and best practices related to mental health, geriatrics, and long-term care needs.
  • Effective communication skills with the ability to interact effectively with individuals and groups through verbal, written, and electronic formats.
  • Attention to detail and the ability to effectively prioritize and execute tasks in a timely manner.
  • Experience in medical record abstraction and clinical data review.

Preferred Qualifications

  • Basic knowledge of Preadmission Screening and Resident Review (PASRR) requirements, including compliance with state and federal regulations.
  • Familiarity with mental health, intellectual and developmental disabilities (IDD), geriatrics, and other conditions commonly requiring long-term care services and supports.
  • Solid understanding of standards of practice related to Level of Care determinations, including those applicable to mental health, geriatrics, and conditions often requiring long-term care placement.
  • Knowledge of psychiatric disorders, as outlined in the current Diagnostic and Statistical Manual (DSM) used in Nebraska, is preferred.
  • Proficient in computer-based software and tools, including Microsoft Office Suite (Word, Excel, Outlook, Teams).
  • Experience working with the geriatric population.

Compensation

The compensation for this role is $39.00 to $41.00 per hour

Based on our compensation program, an applicant's position placement in the pay range will depend on various considerations, such as years of applicable experience and skill level.
