The mission of the Colorado Coalition for the Homeless is to work collaboratively toward the prevention of homelessness and the creation of lasting solutions for homeless and at-risk families, children, and individuals throughout Colorado. The Coalition advocates for and provides a continuum of housing and a variety of services to improve the health, well-being, and stability of those it serves.

The Medical Director provides clinical and administrative leadership for the primary care provider team. This role is responsible for supporting high-quality, trauma-informed, and person-centered care for individuals experiencing homelessness. The Medical Director works collaboratively with interdisciplinary teams to promote integrated care, clinical quality, and operational excellence.
NOTE: In order to ensure that our consumers receive the best possible care the candidate chosen to fill this position will be required to complete our internal credentialing and privileging process prior to a start date. The credentialing and privileging timeline is dependent on the selected candidate's submission of documents necessary for clearance. Please note that a start date will be scheduled once credentialing and privileging is complete.

Essential Job Functions

    • Clinical Leadership and Supervision
    • Supervises and mentors primary care providers, including physicians, nurse practitioners, and physician assistants.
    • Monitors provider productivity, panel management, and overall performance while supporting provider satisfaction, engagement, and alignment with the Coalition's mission and philosophy of care
    • Facilitate recruitment, hiring, onboarding, and ongoing professional development, including continuing medical education and peer review.
    • Address patient care concerns and provider-related issues while fostering a culture of accountability, trauma-informed care, and continuous learning.
    • Collaborate with client access team and associate medical directors to ensure adequate provider coverage and support scheduling across clinical sites.
    • Conducts chart reviews and co-signs documentation as appropriate.
    • Foster a work environment that is marked by respect for others, value inclusiveness, equity, diversity and that exemplifies cooperation and teamwork
    • Clinical Care
    • Provides direct, high-quality primary care services to people experiencing homelessness while demonstrating excellence in clinical practice, documentation, and care coordination.
    • Ensure timely & accurate recording of all clinical/non-clinical services, communications, and tasks in the EHR and other systems as required while maintaining complete up-to-date charts and any future upgrades as directed.
    • Administration
    • Participates in committees and leadership groups including, but not limited to: Credentialing & Privileging, EHR Governance, Clinical Quality, Pharmacy & Therapeutics, and Peer Review
    • Collaborate with Integrated Health teams (such as nursing, pharmacy, client access) to ensure high-quality, coordinated care across all sites
    • Support Continuous Quality Improvement (CQI) initiatives related to primary care performance and assure standards for timely, accurate, and compliant clinical documentation
    • Assures adherence to standards for timely, accurate, and complete clinical documentation, including compliance with medical-legal, billing, and regulatory requirements
    • Ensure compliance with HRSA, CMS, FTCA, and other regulatory requirements.
    • Performs other duties as assigned
Qualifications Summary
    • Provides trauma-informed, high-quality care to people experiencing homelessness, including those served through Medication Assisted Treatment programs and street medicine teams. Offers clinical oversight and mentorship grounded in humility and respect.
    • Builds trust across diverse care teams and fosters a culture of accountability and mutual respect.
    • Participate with a commitment to continuous improvement, health equity, and compliance with HRSA, UDS, and PCMH standards.
    • Mission-Aligned Strategy: Works closely with CCO to align clinical services with CCH's mission to meet patients where they are through integrating medical, behavioral, and social care.
    • Data- and Systems-Minded: Uses EHRs and data tools to improve care delivery, close gaps, and advocate for patient-centered solutions.
    • Experience
    • Minimum of 5 years clinical practice, ideally in primary care (family medicine or internal medicine)
    • Experience in community health, underserved populations, or public health is highly preferred
    • At least 3 years of leadership or supervisory experience
    • Education
    • Medical Degree (MD) or Doctor of Osteopathy (DO), from an accredited school with an active, unrestricted license to practice in Colorado
    • Board Certification and completion of an accredited residency in the physician's specialty, preferably in primary care
    • Current DEA license
    • Master of Public Health (MPH), Master of Healthcare Administration (MHA) or Master of Business Administration (MBA) preferred
