A Brief Overview

The advanced practice practitioner (APP) works in the Infant Single Ventricle Monitoring Program (ISVMP) within the multidisciplinary clinical team to provide individualized patient care through history and physical assessment, ordering and interpreting diagnostic tests, medication ordering, prescribing and management, formulating diagnoses and patient/family education. They promote evidenced based practice by conducting or participating in research studies and academic activities. Participates in quality improvement and research initiatives.

The patient population is one with congenital heart disease or extra cardiac anomalies who requiring staged palliations who are at high risk for morbidity and mortality. Patients may also have other medical conditions such as genetic syndromes and/or prematurity.

Work locations will include the Main Hospital and the Buerger Center. We are looking for a highly motivated individual to advance our program with attention to program growth and development.

Responsibilities

  • Provides clinical care management in the inpatient and outpatient setting
  • Follow course of patients before and after intervention
  • Facilitate hospital discharge and transition to home
  • Serve as liaison between CICU team and ISVMP team for patients expected to be eligible for enrollment in ISVMP.
  • Arrange multidisciplinary review of new patients
  • Participate and independently run telemedicine appointments with patients and families
  • Consultant for outside cardiologists and pediatricians - takes calls related to concerns with ISVMP patients, participates in treatment plan and follow up
  • Provides education about care of fragile ISVMP population to outpatient referrals (i.e., pediatricians, home nurses, etc.)
  • Key contact and CHOP lead with NPC QIC. Responsible for consenting, maintaining enrollment, and data collection
  • Development of on-QI projects in conjunction with work being done at the national level.
  • Review and analysis of ISVMP data for our internal program metrics
  • Maintains all state and national competencies required
  • Nasogastric tube weaning and potential other programs that are developed in the future
  • Write new consult notes and progress notes for patients evaluated
  • Identify and communicate with referring cardiologists at pre-determined time points (birth/admission, post-procedure, weekly update, acute events, transfer/discharge)
  • Orders, performs and interprets diagnostic and therapeutic tests and procedures
  • Orders and prescribes pharmacologic and non-pharmacologic interventions and performs other therapeutic or corrective measures to treat illness and improve health status
  • Collaborates and coordinates care with multi-disciplinary care team, including the nutritionist, feeding team, pharmacy and other subspecialists to deliver evidenced-based care and establish best practices in clinical care
  • Participates in current research studies and quality improvement initiatives on a local and national level
  • Participates in research design, implementation and interpretation.
  • Utilizes advanced assessment skills to manage a cohort of patients. Performs and documents daily assessments
  • Collects and analyzes data and formulates a family-centered plan of care
  • Performs any needed advanced procedures that are listed in the Scope of Practice Document and approved as a part of the credentialing process
  • Evaluates and monitors the effectiveness of the prescribed treatment plan
  • Using the most current evidence, modifies the plan of care based on the effectiveness of the treatment and the patient/family response to the plan
  • Serves as an educational resource to the patient/family, nursing staff, and provider team
  • Acts as a consultant to multidisciplinary health care team members
  • Participates in scholarly activities including research and education
  • Participates in own professional development by maintaining required competencies, identifying learning needs and seeking appropriate educational offerings
  • Participates in the professional development of others. Acts as a preceptor and mentor to support the development of nurses, nurse practitioners, students, and other health care providers.
  • Demonstrates a commitment to patient safety by speaking up about safety issues and changing practice to enhance safety. Shares errors or potential errors through system reporting mechanisms. Demonstrates shared accountability for safe practice.

Education

  • Required: Master's degree- Nursing

  • Preferred: Doctorate- Nursing

Skills and Abilities

  • Ability to work in a team setting with multiple responsibilities

  • Ability to use computer and electronic medical records effectively

Licenses and Certifications

  • Required (upon hire in Pennsylvania): Registered Nurse and Certified Registered Nurse Practitioner (CRNP) (Pennsylvania) - Pennsylvania State Licensing Board

  • Required (upon hire in New Jersey): Registered Nurse and Advanced Practice Nurse (APN) (New Jersey) - New Jersey State Licensing Board

  • Required (upon hire): Specialty Certification as a Nurse Practitioner in area of education/subspecialty

  • Required (within 3 months): Prescriptive Authority - Pennsylvania State Licensing Board or New Jersey State Licensing Board (depending on location)

SALARY RANGE:

$104,600.00 - $138,600.00 Annually

Salary ranges are shown for full-time jobs. If you're working part-time, your pay will be adjusted accordingly.
