SHIFT: Day (United States of America)A Brief Overview The Progressive Care Unit (PCU) services 25 beds and patients with long-term care management needs. Many patients supported on this unit require technology dependence of tracheostomy and ventilation support with diagnoses including chronic lung decease of prematurity, pulmonary hypertension, cardiac anomalies, multiple congenital anomalies, neurologic injury and short gut. The Social Worker in this area serves as the expert in connecting families to vital resources, coordinates medical team meetings to assist with care planning and family engagement, assists families with navigating secondary Medicaid and SSI processes and partners with families seeking information for long-term care facilities. The PCU Social Worker is a part of the multidisciplinary team and serves as an advocate for helping families navigate diagnostic outcomes. The Social Worker provides assessments to determine protective factors for the families related to their care as well as potential stressors to address through internal and external resource referrals. Social Work Professionals with experience within a long-term pediatric and adolescent care setting as well as familiarity with NJ MLTSS, Medicaid Waivers, Life Sharing Process and MBA Opens Doors Foundation are highly preferred. SALARY RANGE: $67,500.00 - $86,060.00 AnnuallySalary ranges are shown for full-time jobs.
