Overview
The Behavioral Health Case Manager works with the behavioral health multidisciplinary team to provide care for the Responsibilities Essential Duties and Responsibilities * Works with patients on the inpatient unit, emergency room,medical floors and day treatment programs. * Consults and coordinates care of patient with the physician/psychiatrist, RN, social worker/therapist and other agencies. * Assesses treatment planning and progress. * Prepares treatment plan and goals for patients. * Maintains pertinent patient data in the EMR in accordance with state and federal laws and hospital policy. * Provides y psychoeducational groups. * Arranges for disposition and follow up recommendations. Education and Experience Master's Degree in a mental health, behavioral health field with documented clinical course work. - Bachelor's will be considered for lower level case manager positions. Working toward licensure is preferred. Approved CPR/BLS within 90 days after hire.
