Job Summary

The Youth Transition Specialist is responsible for designing, coordinating, and delivering pre-employment transition services (Pre-ETS) to high school and transition-aged youth and/or students with disabilities. This includes developing individualized and group-based programming that supports career exploration, workplace readiness, self-advocacy, and post-secondary planning in alignment with grant objectives and compliance requirements.

Primary Duties

Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions. These duties and responsibilities will be rated on the Annual Performance Review.

  • Conduct engaging, relevant, informative, and dynamic group sessions with high school students with disabilities, leading activities that cover the range of Pre-Employment Transition Services: 1) Job Exploration Counseling; 2) Workplace Readiness Skill Building; 3) Work-based Learning Experiences; 4) Postsecondary Educational Counseling; and 5) Self-Advocacy Skill Building. Customize activities to the unique needs and strengths of each group while maintaining fidelity the outcomes and objectives of the curriculum.

  • Seek regular formal and informal feedback from participants and their support team members regarding pre-employment transition services provided to learn satisfaction and to update curriculum topics and activities as needed.

  • Lead outreach and recruitment efforts for attendees for new groups, including disseminating outreach flyers; holding information sessions; and answering questions as needed from referring partners, interested participants, and family members.

  • Develop and maintain positive relationships with partner agencies, school personnel, businesses, educational institutions, other departments and/or vendors to enhance menu of services, continuum of care, and the number of employment transition opportunities.

  • Provide general case management, maintaining excellent communication with participants, family members, funding and referring agencies, schools, and other partners, sending monthly requests for authorizations; submitting accurate and detailed monthly reports on time; and assisting with the billing process.

Additional Responsibilities

  • Complete accurate and timely monthly billing reports, participant reports, case notes, and other supporting documentation

  • Conduct tours and informational interviews with representatives from employers and educational programs to choose the most relevant and highest quality settings for participation in pre-employment transition group activities.

  • Provide consultation, guidance, and disability awareness training to employers and educational program representatives to foster positive partnerships.

  • Communicate regularly with both internal and external Directors, Supervisors, Rehabilitation Counselors, and/or any other relevant individuals regarding all pre-employment transition activities and progress of participant with career and education exploration, building of work readiness and self-advocacy skills, and work-based learning experience participation.

  • Attend relevant funding and referring partner meetings to discuss participants, receive feedback and updates, and market services with ServiceSource to increase number of referrals

  • Take participants on tours of employment and educational settings.

  • Position sometimes requires work outside of the normal business day which mirror the needs of the assigned participants. Therefore, the employee must be able to work a flexible schedule, which may include weekends and evenings.

  • Perform other responsibilities as assigned.

Qualifications: Education, Experience and Certification(s)

  • Associates Degree (AA/AS) from an accredited college or university required. Relevant degrees may include Special Education, Psychology, Social Work, Sociology, Human Services, Rehabilitation Counseling preferred, or in lieu of degree a minimum (4) years of relevant experience.

  • Two years of experience working with individuals with disabilities required.

  • Experience working with transitioning students and/or young adults with disabilities, or other relevant, transferrable work experience, required.

  • Prior experience public speaking, training, or teaching on a one-to-one basis is preferred.

  • Prior experience in developing partnerships with employers and/or postsecondary educational institutions is preferred.

  • Pass and maintain Department of Human Services background clearance is required.

  • Valid driver's license and/or access to reliable transportation to perform work-related travel required. Eligible drivers must have a good motor vehicle record (MVR).

Knowledge, Skills, and Abilities

  • A high-energy, dynamic self-starter and leader with excellent communication and interpersonal skills who is able and eager to provide broad support to high school students with a variety of disabilities.

  • Ability to follow a curriculum to train and teach in a group setting in an engaging and informative manner and customize activities as needed.

  • A well-organized and self-directed team player.

  • An individual who can relate to people at all levels of an organization.

  • Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists.

  • Must have knowledge of employment transition, supported employment, and job training issues for persons with disabilities.

  • Proficiency in Microsoft Office skills, with the ability to become familiar with corporate specific programs and software.

Physical Demands

Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.While performing the duties of this job, the employee is regularly required to sit, walk and stand; talk and hear. The employee may occasionally lift or move office products and supplies, up to 25 pounds. Specific vision abilities required by this job include close vision, distance vision, depth perception, and ability to adjust focus.

Work Environment

The primary work environment is an office setting with moderate noise and regular interruptions. The environment may also include a variety of specialized learning settings. The employee will be visiting a wide variety of community settings including but not limited to restaurants, stores, recreation facilities, volunteer sites, schools and employer worksites which can vary with regards to noise level (up to loud) and weather/temperature conditions.
