Position Summary:
We have an exciting opportunity to join our team as a Licensed Practical Nurse -Hemodialysis- -Main St-Full-Time- Day Shift - Hemodialysis-Main St - Full-.

In this role, the successful candidate Under the supervision of the registered nurse, participates in the planning, implementation and evaluation of nursing care. Performs direct, technical nursing care functions in accordance with the physician treatment plan and the patient's plan of care. Observes the patient, notes patient's responses, records pertinent information and reports significant information regarding the patient to the registered nurse. Works collaboratively with the interdisciplinary team to meet the holistic needs of the patient. Participates in the orientation process of new employees.

Job Responsibilities:

  • Assists the registered nurse with assessment by collecting patient health data by involving the patient, family and other health care providers.
  • Participates in the analysis of the data and the identification of expected outcomes.
  • Collaborates with the patient, family and appropriate healthcare providers in developing the plan of care.
  • Demonstrates awareness of the specific needs of each age group or culture served as well as those patients with disabilities.
  • Recognizes the patient's comfort/pain management needs and provides measures to facilitate comfort.
  • Organizes work load and uses time efficiently and effectively.
  • Provides direct technical nursing care to patients by performing a variety of treatments such as wound care, dressing changes, and capillary blood glucose
  • monitoring in compliance with policy/procedure.
  • Assists the patient with activities of daily living and encourages self-care as appropriate.
  • Observes patients for physical and emotional reaction to medications or treatments reporting any untoward changes to the appropriate medical and/or nursing personnel.
  • Observes the patient and records pertinent information.
  • Reports significant symptoms, reactions or changes in the patient's condition in a timely fashion.
  • Assists with patient and family education as outlined in the patient plan of care and communicates outcome.
  • Demonstrates a working knowledge of the principles, policies and procedures specific to the assigned nursing service.
  • Maintains a clean, safe and orderly patient environment.
  • Participates in the evaluation of the patient's response to the plan of care by communicating with the Registered Nurse and other members of the health care team.
  • Collaborates with the Registered Nurse in prioritizing or setting new goals to obtain the desired patient outcome.
  • Demonstrates knowledge regarding drug administration, incompatibilities, contraindications and adverse effects.
  • Complies with medication policy and procedure for order transcription/verification, and drug administration.
  • Identifies the patient prior to medication administration by checking the identification arm band.
  • Administers medication via approved routes utilizing outlined techniques.
  • Completes required administration documentation on the medication administration record.
  • Seeks out appropriate resources when uncertain regarding an aspect about medication administration.
  • Administers intravenous therapy in accordance with the policy and procedure.
  • Completes the intravenous documentation requirements regarding initiation of site, site monitoring and intake/output data.
  • Demonstrates awareness and participates in activities that support customer satisfaction.
  • Seeks out and utilizes the Nurse Manager or Nursing Administrative Supervisor to address issues that require their assistance to resolve.
  • Works collaboratively with management to improve the environment of care for patients, family and personnel
  • Participates in the evaluation of the quality and effectiveness of nursing practices and supports measures to improve practice.
  • Participates in the orientation process for new employees.
  • Interacts effectively with the staff on the unit and in other departments.
  • Maintains a professional appearance as outlined in organizational guidelines.
  • Maintains current and ongoing knowledge in the field to promote professional competence.
  • Seeks out and participates in learning situations such as educational programs, seminars, department meetings, or committee participation for personal and professional development.
  • Complies with staff meeting attendance requirements.

Minimum Qualifications:
To qualify you must have a Knowledge of nursing theory and practice acquired through completion of a training program from an approved, accredited School of Practical Nursing
Current New York State license as a Licensed Practical Nurse
Interpersonal skills necessary to effectively interact with patients and their families that demonstrate recognition of cultural diversity
Practice reflects customer service excellence
Ability to communicate in verbal and written format
Basic analytical skills in order to gather data and interpret the information to address patient care needs.
BLS certification, de-escalation.
Maintains knowledge and proficiency in the clinical area and age group served
Ability to frequently lift/carry more than 75 lbs.
Ability to frequently push/pull more than 100 lbs.
Ability to frequently bend, squat, kneel, climb steps/ladder and reach above shoulders..
