Job Summary:

Nature of Work

The Case Manager will be faculty of the Department of Psychiatry, integrated within the Department of Neurology, Functional Neurological Disorders (FND) Program. The FND Program is an outpatient, multidisciplinary treatment program comprised of neurologists, psychiatrists, social workers, nurses, case managers, allied health professionals and administrative staff. The FND Program boasts local, national, and international recognition for their highly collaborative, innovative care model to support individuals with FND. This position will have the opportunity to participate in clinical research, quality, and process improvement initiatives in addition to their clinical work.

About the Functional Neurology Program: The FND Program is located in the UCHealth Hospital Outpatient Pavilion on the University of Colorado Anschutz Medical Campus in Aurora, Colorado. The service line began in 2016 and has since treated over 2,000 patients. Previously, the FND Clinic has only served individuals with functional seizures (psychogenic non-epileptic seizures) but recently received funding to expand their operations to care for individuals with functional movement symptoms.

Treatment is delivered using a hybrid model, including both virtual and in-person appointments. As a member of an academic medical center, this position will support both a clinical and academic mission and will have the opportunity to participate in quality improvement and clinical research initiatives. Further, the position will have the opportunity to improve UCHealth, University of Colorado and greater community's knowledge of FND through educational initiatives.

Reports to:

This position will be appointed in the Department of Psychiatry in the Adult Division- Integrated Services per policy, the Director of the FND Clinic exhibits ultimate responsibility for this position's professional and clinical conduct. Professional development will be supported by the Department of Psychiatry.

Within the FND Program clinical oversight will be provided by the Clinical Director of Behavioral Health Services.

Key Responsibilities:

  • Principal Professionals are responsible for exercising discretion, analytical skill, personal accountability and responsibility in a wide range of areas including academic, administrative, managerial and student services functions. Work involves creating, integrating, applying and sharing knowledge directly related to a professional field. Work is performed at an advanced or expert level.

  • Senior Professional are responsible for exercising discretion, analytical skill, personal accountability and responsibility in a wide range of areas including academic, administrative, managerial and student services functions. Work involves creating, integrating, applying and sharing knowledge directly related to a professional field. Work is performed fully independently.

  • Intermediate Professionals are responsible for exercising discretion, analytical skill, personal accountability and responsibility in a wide range of areas including academic, administrative, managerial and student services functions. Work involves creating, integrating, applying and sharing knowledge directly related to a professional field.

  • Assessment and Care Planning: Conducts comprehensive assessments of patients' FND, behavioral health, psychosocial needs, and risk factors when appropriate. Develops individualized care plans in collaboration with patients, FND Program multidisciplinary team and outside care providers. Assists with formulating patient-centered goals that address FND recovery, mental health needs, social determinants of health, and barriers to care.

  • Care Coordination: Assists with overcoming barriers to attending visits in the FND Program. Coordinates with medical providers, mental health professionals, social workers, and other healthcare teams to implement care plans. Facilitates referrals to community resources, support services, and behavioral health programs (e.g., therapy, substance use treatment). Monitor patient progress and adjust care plans, as necessary. This includes but is not limited to contacting patients to follow up on progress, filling out FMLA paperwork, etc.

  • Patient Education & Advocacy: Educates patients and families about available treatment options, coping strategies, and resources for managing FND. Advocate on behalf of patients to ensure they receive appropriate care, services, and support. Assist patients in accessing benefits, financial resources, and community services (e.g., housing, employment assistance).

  • Compliance & Quality Improvement: Ensures all care coordination activities comply with organizational policies, HIPAA, and other relevant regulations. Participates in quality and process improvement initiatives aimed at enhancing FND Program services and patient outcomes.

  • Budget/Resource Management: Informs FND Program Director and Business Manager of behavioral health service line budgetary and resource needs.Works closely with FND Program faculty and staff to develop sustainable resource management database. This includes but is not limited to designing and organizing resources.

  • FND Resource Collaborative: Works closely with FND Program Clinic Coordinator to further develop and maintain statewide, FND Resource Collaborative. This includes identifying new members, maintaining up-to-date information on current members, and communicating when member education is needed.

  • Community Engagement/EducatioCollaborates with FND Program faculty and staff to support or conduct local and national educational engagement activities and initiatives. This includes but is not limited to community educational webinars, development of educational materials, etc.

Additional responsibilities may be assigned as needed.

Work Location:

Hybrid - this role is eligible for a hybrid schedule rotating between working from home and in-person. The current distribution of effort is 1.0 FTE, which includes 40 hours per week. Clinical time includes direct patient care. Administrative time includes but is not limited to coordination of care, educational activities, scholarly work, and supervision of trainees.

Principal Professional

  • Bachelor's degree from an accredited institution.

  • 3-5 years of experience* in clinic case management, or highly related work.

  • Prior experience working with families and caregivers.

Senior Professional

  • Bachelor's degree from an accredited institution.

  • 2 years of experience* in clinic case management, or highly related work.

Professional

  • Skills in risk assessment, including suicide and violence risk assessment.

  • CITI Certifications will be arranged prior to appointment.

  • Experience caring for functional neurological disorder (conversion disorder) or other somatic symptoms.

  • Prior experience delivering trauma-informed care.

  • Prior experience working with families and caregivers.

  • Prior experience working in multidisciplinary teams.

  • Comfortable with public speaking.

  • Return phone calls or emails from faculty and staff patients.

  • Chart patients visits using an electronic medical record.

  • Triage and refer patients as needed to providers who can offer an appropriate level of care.

  • Participate in quality- and performance assessment projects.

  • Comfortable with outreach to new resources and conducting advocacy work with community stakeholders.
