Summary Job Description: 24 Hours Weekly, PT, Nightshift (7pm-7 am) The Licensed Practical Nurse (LPN) functions as an essential on-shift team member, working under the delegation of a Registered Nurse (RN) to deliver direct, patient-centered care that meets the needs of patients, families, and coworkers. The LPN performs focused nursing assessments, contributing to the RN's comprehensive care planning through appraisal of a patient's current status and needs. Responsibilities include maintaining effective communication with physicians and the healthcare team, participating in hourly rounding, utilizing electronic medical records efficiently, and ensuring patient safety within the LPN scope of practice. The LPN collaborates with the interprofessional team by updating patient records, communicating care needs, and escalating concerns when appropriate. They actively participate in quality improvement initiatives, including daily huddles and efforts to reduce hospital-acquired infections and falls. The LPN also implements physician orders within scope and ensures timely review and execution, seeking RN support for first-dose IV medications. Medication administration includes oral, topical, subcutaneous, intramuscular, gastric tube, and urinary routes, with all documentation completed per policy.
