Job Title: Licensed Practical nurse Location: Camp Lejeune, NC 28547 Duration: 12 months to long term Shift: 07:30 AM - 16:30 PM (Mon - Fri) Role & Responsibilities: Reach out to patients to review medication titration, ensuring clear communication and adherence to treatment plans. Take ownership of your tasks while ensuring high standards of accuracy and professionalism. Maintain precise, detailed documentation of patient interactions in compliance with HIPAA and Cadence's clinical protocols. Use your expertise to guide patients in understanding disease-specific medications and how they relate to their care plans. Show up on time and consistently meet deliverables, ensuring patient care and service are never compromised. Provide timely, clear, and empathetic communication with patients and healthcare providers. Manage your responsibilities independently while maintaining high performance and efficiency. Documents Required: Active LPN License, BLS/CPR Certification.
