The Clinical Field Preceptor is responsible for supporting the orientation, training, and development of new field clinicians. This role provides hands-on instruction in the field, reinforces clinical and compliance standards, and ensures staff are equipped to deliver safe, effective, and compassionate care in accordance with the Physician Plan of Care, company policies, and professional standards of practice.

  Serve as a mentor and trainer for new field clinicians, ensuring competency in patient care and documentation.

  Provide clinical instruction in the field, demonstrating best practices in home health visits.

  
  
• 
  

  Collaborate with the Director of Professional Services/Clinical Manager to assess skills, identify learning needs, and provide constructive feedback.

  
  
• 
  

  Ensure compliance with agency policies, federal/state regulations, and Joint Commission standards.

  
  
• 
  

  Document training activities, clinical performance, and areas for improvement.

  
  
• 
  

  Participate in interdisciplinary meetings to help integrate new staff into the care team.

  
  
• 
  

  Support ongoing education and professional growth for field clinicians.

  
  

• 
  

  Current Florida license as an RN, PT, or other qualifying clinical discipline

  
  
• 
  

  Minimum 2 years of Medicare home health experience required

  
  
• 
  

  Strong knowledge of OASIS and home health regulations

  
  
• 
  

  Excellent mentoring, teaching, and communication skills

  
  
• 
  

  Strong ability to build rapport with staff, patients, and families

  
  
• 
  

  Proficiency with Kinnser/WellSky or similar EMR systems preferred

  
  
• 
  

  Current CPR, Level 2 background screening, and recent TB/CXR required
