Job description
Position Purpose:
The Support Coordinator - Community Living has the primary responsibility of support the Quality Assurance Specialist to ensure quality services are provided and the level of care and dignity individuals in VersAbility's homes and residential programs is maintained at optimal levels. This directly affects the image and reputation of VersAbility in the community and by extension, on the communities' perception of people with disabilities. Primary Duties: *Conduct pre-screenings and coordinate/facilitate admissions and discharges for all CL programs. Maintain waiting list and referral list for all CL services. May include facilitating Discharge meetings as needed. *Maintain active caseload for residential and day support ICF-ID Individual Service Plans (ISPs). Responsible for ISP development, implementation, Person-Centered planning and review monitoring *Plan, coordinate and facilitate quarterly, annual, and as needed, Interdisciplinary Team (IDT) meetings and ensure each is documented properly. *Provides accurate and timely documentation as required for DBHDS, Medicaid Waiver, Department of Social Services, Social Security, VDH, and ICF-ID licensing requirements. This will include monthly, quarterly and/or yearly reports. *Establish and maintain process for recruitment of viable candidates for CL programs (with Marketing Department). *Serves as liaison with families, friends and other service organizations to ensure individuals' needs are met financially, physically and socially, plan quality leisure activities and experiences for program participants. *Develop and submit audit monthly schedule and maintain document of audits on file. Inspect residential and day programs for cleanliness, safe environment and proper documentation. *Read, interpret, implement and understand policies, licensures, regulations, program requirements and state of the art methodologies of the treatment of persons with developmental and other disabilities. *Maintain files of residents financial and medical resources, assist Finance Dept. with facilitating insurance claims and correcting inaccurate medical billing. *Provide guidance and support for community integration efforts and HCBS Rights Compliance. *Assists Quality Assurance Specialist to ensure residential and day programs are continually in compliance with the standards and regulations of the Department of Licensure, DBHDS, Medicaid Waiver, CMS/ICF-ID, Department of Health, CARF, HCBS, Department of Social Services and any other applicable bodies. *Assist with staff training when requested. *Assist in the implementation of new programs and services as needed. *Assist with carrying caseload in absence of other support coordinators. *Assist with assuming back-up coverage in absence of Quality Assurance Specialist. *Other related duties as assigned. Education: Bachelor's degree in human services, psychology or a related field required. Must be able to maintain CPR, CPI, first aid and medication administration certifications on an ongoing basis. Must have a safe driving record and a valid state driver's license. Experience: Two years' experience working with adults with development and other disabilities, - or experience interpreting and implementing regulations, - or designing and implementing programs, - or other equivalent experience that would prepare for this position.
