Your team will help connect members to housing, medical care, benefits, and ongoing behavioral health treatment - ensuring stability and continuity of care. You'll work within a collaborative, mobile team model, combining clinical insight and compassionate fieldwork to support some of San Diego County's most vulnerable residents.
