Job Summary

The Specialty Pharmacy Hub Clinical Pharmacist is expected to provide clinical patient care for a variety of specialty clinical programs at UCDH Specialty Pharmacy. This role supports patients in receiving appropriate outpatient and specialty pharmacy medications. As a staff pharmacist, they are an integral member of the health care team, responsible for patient education and consultation, drug therapy monitoring, distribution, and drug information inquiries. This position fosters excellent interdepartmental relationships and provides quality and cost-effective pharmaceutical care to all patients.

A pharmacist in this role is expected to adapt to varying clinical practice areas, assist with transitions of care, provides education to healthcare professionals and patients, develop workflows and supports protocol development, and ensures compliance in practice and documentation with institutional, regulatory, and accreditation standards. The Specialty Pharmacy Hub Clinical Pharmacist is involved with precepting pharmacy students and residents, conducting clinical research, participating in committees, task forces and work groups while fostering excellent interdepartmental relationships.

Other duties may include providing drug information and recommendations, assisting with insurance/prior authorization needs, supporting drug dispensing and distribution activities, compounding activities, maintaining pharmacy robotics, inventory management, prescription billing, processing payments for prescriptions, providing medication consultations, providing immunizations, serving as a preceptor to pharmacy students and residents, and providing training to other personnel. As a part of the outpatient pharmacy service line, Specialty Pharmacy staff are responsible for appropriate prescription billing to multiple payers including third party insurances, Medicaid, Medicare D and Medicare B plans. Our pharmacies maintain compliance with multiple accrediting bodies and with State and Federal Laws and Regulations.

Minimum Qualifications - For full consideration, applicants are encouraged to upload license and/or certification if required of the position.

  • California Pharmacist License eligible and possess a valid license at the time of hire
  • Doctor of Pharmacy Degree from an accredited college or university, or equivalent training.
  • Completion of a Post Graduate Year One (PGY-1) Pharmacy Practice or Specialty Residency, or 3 years of relevant pharmacist experience
  • Recent outpatient/specialty pharmacy or ambulatory clinical pharmacist work experience
  • Clinical pharmacist experience with medical terminology, lab data, optimal drug therapy, and collaboration with healthcare providers, clinic staff, and other members of a patient's care team.
  • Excellent ability to effectively communicate with and educate/counsel patients about their medication therapy.
  • Computer skills that facilitate clinical patient data or medication order entry, chart documentation in the electronic medical record (EMR), and use of other software programs.
  • Recent working knowledge of prior authorization, appeals, and pharmacy benefit structure and formulary requirements
  • Demonstrated ability to obtain needed information from a patient or patient's chart to create medication management plans, identify medication-related problems in patients, and communicate with to provide recommendations to the patient's provider.
  • Strong written and oral communication skills to clearly and concisely provide information, explanation and instructions to, and to elicit information from others with varying levels of ability to understand, including individuals who are ill or distressed, or who may not speak English.
  • Knowledge of current ambulatory care guidelines (i.e. NCEP, JNC, ADA).
  • Demonstrated ability to triage and prioritize to provide patient care.
  • Decision-making skills to independently exercise delegated authority to take action, and to recognize when it is appropriate to seek assistance from others when limits of delegated authority have been reached.

Preferred Qualifications

  • Board of Pharmacy Specialties (BPS) or Certified Specialty Pharmacist (CSP) Certification
  • Completion of a Post Graduate Year Two (PGY-2) Pharmacy Practice or Specialty Residency
  • Pharmacy work experience in outpatient/retail or specialty pharmacy
  • Experience in overseeing technicians and establishing priorities of workflow for timely patient care on a daily basis.
  • Experience with URAC (or similar standards) for Specialty Pharmacy or other pharmacy accreditation
  • Experience in working with and directing health care professionals.
  • Experience in precepting and evaluating residents and students.
  • Ability to navigate third party reimbursement programs and resolve insurance issues.
  • Problem-solving skills to independently recognize and define a problem; identify the resources available to help solve the problem; create and implement viable solutions; and follow through to ensure the problem is resolved to the satisfaction of all parties.

We also have the largest pharmacy residency program in the country with 46 total residents in 13 different programs.

The Department of Pharmacy offers comprehensive pharmaceutical services (unit dose, IV admixtures, clinical services, infusion pumps, and teaching) throughout the health system complex 24 hours a day, 7 days a week. The pharmacy services encompass inpatient pharmacy services as well as an expansive footprint in the ambulatory care space where we have clinical pharmacists embedded within our clinics and specialty care locations. Our operational teams support the health system enterprise with three inpatient pharmacies, five outpatient pharmacies, two infusion pharmacies, a home infusion service line, and the full complement of pharmacy purchasing, stores, and distribution.
