UNM Medical Group, Inc. is hiring for a Community Health Worker to join our Street Medicine team. This opportunity is a full-time day shift opening located in Albuquerque, New Mexico. We are seeking a dedicated and compassionate Community Health Worker to join our Mobile Medical team - a program that brings healthcare directly to individuals experiencing homelessness and living in unsheltered environments. This is a unique opportunity for a CHW who is passionate about health equity, social justice, and walking alongside people on their path to better health. In this role, you won't be based in a clinic or office. Instead, you'll be out in the field - on sidewalks, in encampments, and in other non-traditional spaces - helping connect people to care, resources, and support. You'll play a critical role in building trust, reducing barriers, and advocating for those who are often left out of traditional healthcare systems. Required: Possession of a valid unrestricted NM driver's license. Minimum $44,604 - Midpoint $55,766* *Salary is determined based on years of total relevant experience. *Salary is based on 1.0 FTE (full time equivalent) or 40 hours per week. Less than 40 hours/week will be prorated and adjusted to the appropriate FTE. Summary Under general supervision, as part of a multi-disciplinary team, provides services relating to improving retention in care for specialized subsets of patients. Responsible for building patient relationships that encourage disease management and connection with healthcare providers. Conducts prevention intervention programs and shares responsibility for maintaining, enhancing and presenting health information and resources related to chronic disease and other relevant health issues. May perform a range of administrative functions required to ensure efficient program operation. Minimum Job Requirements of a Community Health Worker: High School diploma or GED. Must complete New Mexico State CHW Certification within 1 year of employment. Completed degree from an accredited institution that are above the minimum education requirement may be substituted for experience on a year for year basis.
