Position Summary
The Occupational Therapist provides skilled, goal-oriented care to patients and their families through individualized treatment plans that promote functional independence and improve quality of life. This role manages a patient caseload, delivers evidence-based therapeutic interventions, and coordinates interdisciplinary care while supervising Certified Occupational Therapy Assistants. The position supports regulatory compliance, quality improvement initiatives, and contributes to the organization's clinical excellence and community reputation.

Key Responsibilities
* Develop and implement individualized occupational therapy plans of care, including comprehensive initial and ongoing assessments.
* Deliver therapeutic treatments and adaptive strategies that align with physician orders, regulatory standards, and patient goals.
* Coordinate care with patients, families, physicians, and the interdisciplinary team to ensure continuity and quality of services.
* Supervise and document care plans for Certified Occupational Therapy Assistants, including supervisory visits.
* Participate in interdisciplinary team meetings, quality improvement activities, and professional development.
* Maintain accurate and timely clinical documentation in compliance with agency standards and billing requirements.
* Promote patient safety and rights by adhering to HIPAA, infection control protocols, and emergency procedures.

Qualifications
* Current, active Occupational Therapist license in the state of practice and in good standing.
* One year of occupational therapy experience; home health experience preferred.
* Proficiency in OASIS and home health regulations.
* Strong clinical judgment, documentation accuracy, and communication skills.
* Ability to collaborate effectively within a multidisciplinary healthcare team.
* Commitment to patient-centered, compassionate care.

Work Environment
This role involves providing care in patients' homes, assisted living facilities, or clinical settings. The position requires frequent standing, walking, and occasional lifting or transferring of patients using proper body mechanics and assistive devices. Routine travel within the service area is required.

OSHA Category 1
This position involves routine exposure to blood, body fluids, communicable diseases, or other potentially infectious materials. Universal precautions, infection control procedures, and personal protective equipment are required.
