Responsible for overseeing the daily operations of clinical care coordination and case management functions. Ensures the delivery of high-quality patient care by optimizing workflows, managing staff performance, and facilitating communication between care teams. Works to streamline processes, improve patient outcomes, and ensure compliance with hospital standards and regulations. Education Bachelor's Degree Nursing required or Bachelor's Degree Healthcare Administration required or Bachelor's Degree Related Field of Study required Can this role accept experience in lieu of a degree? No Licenses and Credentials Registered Nurse [RN - Massachusetts] Experience Clinical experience in case management or care coordination, 3-5 years required Experience in a supervisory role, 1-2 years required Knowledge, Skills and Abilities Knowledge of healthcare regulations, care coordination best practices, and patient care standards is required. Strong leadership and team management skills to effectively guide clinical staff. Excellent communication and interpersonal skills for collaborating with healthcare teams and patients. Analytical skills to monitor and improve clinical operations and patient outcomes. Problem-solving abilities to address challenges in care coordination and operational workflows. Proficiency in healthcare technology and case management systems to optimize workflow efficiency.
