Job Description
Elwyn seeks a Case Manager to maintain regular contact and visits with families on their assigned caseload to work on the family's goals based on a plan created in collaboration with the family and the Department of Human Services (DHS) worker, when applicable, based on the reason for the referral/complaint. The Case Manager works with families to provide support and counseling in the areas of appropriate parent-child interaction. Family Empowerment Services case managers provide bi-monthly home visits to promote family stability and well-being, enhance protective factors, reduce family risk factors, increase community supportive networks/linkages and increase a family's ability to address their own needs. ESSENTIAL DUTIES & RESPONSIBILITIES
- Upon referral from DHS, creates a plan for each family assigned to caseload including but not limited to interventions to address concerns identified in the DHS-provided case summary
- Collaborate with families on assigned caseload to set and achieve goals to alleviate stress interfering with positive parent-child relationships including but not limited to assisting parent(s) in accessing and using services and resources related to medical, behavioral health, housing, and income supplementation
- Maintain regular contact with families on caseload by visiting homes to check-in and provide support
- Assess family using the Family Advocacy and Support Tool (FAST), a standardized tool to determine the family's level of risk factors
- Establish and maintain professional relationships with families using family-centered, strength-based approach while providing supportive services
- Provide support and counseling in the areas of appropriate parent behavior to help parent(s) increase their ability to problem-solve and advocate for themselves and their children
- Observe parent(s), children in their homes and communicate any concerns about child safety promptly to supervisor and appropriate agencies
- Engage primary caregiver(s) and other key participants in active, change-oriented treatment by identifying and overcoming barriers to engagement and attainment of identified goals
- Maintain clear and concise documentation of treatment interventions and case development in accordance with reporting and documentation regulations and agency policies
- Provide services based on the family's schedule, including provision of services during "non-traditional hours" which include evenings and weekends if necessary
- Adhere to professional code of ethics and follow existing federal and state guidelines and laws pertaining to mandated reporting procedures
- Complete all program paperwork to agency standards and submit according to determined time frames
- Enter demographic data and progress notes for each family on caseload into the DHS Community Based Prevention Services (CBPS) database.
- Perform other duties as assigned
QUALIFICATIONS & SKILLS
- Bachelor's degree in social work, or related human services field
- Two (2) years' experience in social work or related field specifically working with case management and child welfare, children and youth, juvenile delinquency, and/or families
- Experience working with children birth to 17 years of age and their families living in urban communities
- Knowledge of normal child growth/development and appropriate parent-child relationships
- Experience using standardized assessment tools; FAST preferred
- Knowledge of community resources
- Non-profit human service experience highly preferred
- Demonstrated ability to work effectively as part of a team
- Ability to work in a fast-paced environment and manage and prioritize multiple, often competing, priorities
- Demonstrated strong attention to detail
- Demonstrated strong time management and organizational skills
- Demonstrated excellent judgment with the ability to solve problems and make decisions in collaboration with supervisor
- Demonstrated ability to anticipate future needed changes or identify problem areas and take effective actions
- Must possess excellent customer interaction, collaboration, presentation, and written and verbal communication skills
- Demonstrated basic experience with Microsoft Office applications, including Word, Excel, Outlook, and PowerPoint; Access, and report-writer experience preferred
- Must have current, valid driver's license in state of residence, three (3) years driving experience in the United States, and acceptable driving record
