Music Therapist - Hospice

HealthPartners is currently hiring for a Music Therapist - Hospice. The music therapist provides services to clients/families by assessing, developing, and implementing a music therapy plan of care, which is coordinated with a hospice interdisciplinary team.

Required Qualifications:

  • Bachelor's degree at a college or university accredited by the American Music Therapy Association and completion of an internship.
  • Current board certification from the Certification Board for Music Therapists.
  • Demonstrated expertise in program development.
  • Demonstrated ability to establish rapport quickly with people of all backgrounds.
  • Excellent listening and oral and written communication skills.
  • Excellent people skills and ability to establish strong working relationships with interdisciplinary teams.
  • Excellent organizational skills.
  • Current MN Driver's License and daily access to reliable, insured vehicle.
  • Ability to work independently with minimal supervision.

Preferred Qualifications:

  • Master's degree at a college or university accredited by the American Music Therapy Association and completion of an internship.
  • Two (2) years' experience in developing music programs and plans of care, preferably in a Hospice/Home Care setting.

Hours/Location:

  • M-F (4 days per week with flexibility); core business hours (8:00am-4:30pm)
  • During assigned shifts, frequent travel is required across the Minneapolis/St. Paul metropolitan area and surrounding communities.

Responsibilities:

  • Participates in the development, implementation, and evaluation of policies and procedures regarding care plans for hospice patients and families.
  • Conforms to Medicare/Medicaid guidelines.
  • Provides music therapy services that may consist of but not be limited to:
    • Initial assessment and reassessment of patient/caregiver needs relative to a music plan of care.
    • Develops and implements a music plan of care addressing patient/caregiver needs.
    • Observes, evaluates, and reports music reactions to interventions as well as changes in patient's physical or emotional condition.
    • Trains/instructs patients and caregivers in music therapy techniques and promotion of these techniques.
  • Provides in-services to maintain teams' awareness of the role of the music plan of care in addressing patient/caregiver needs.
  • Participates in interdisciplinary conferences, staff meetings, educational activities, and in-services as requested.
  • Communicate with patients and families, Hospice staff, referral sources, physicians, and others.
  • Performs other related duties as required.
