We are excited to launch a new Milwaukee County team and are seeking Care Coordinators to support children with developmental, physical, or severe emotional disabilities through the Children's Long Term Support (CLTS) Waiver Program. Role Overview As a CLTS Care Coordinator, you will serve as the primary point of contact for families, helping them access services that keep children safe and thriving in their home and community.
