The compensation offered to the candidate selected for the position will depend on several factors, including the candidate's educational background, skills and professional experience.

Job Overview:

The Patient Navigator provides care coordination for pediatric patients with

rehabilitation needs and/or acute, chronic or life-altering diseases. They serve as an

active liaison between patients, families, medical providers/sub specialists, therapists,

admissions team members, administrators, insurance companies and other community

providers/agencies within a patient/family-centered care model.

Qualifications:

Required:

  • Bachelor's degree with experience in care coordination.
  • Experience in Pediatrics.

Scheduling Requirements:

  • Per Diem Day

Essential Functions:

* Coordinates care for babies, children and young adults across a care continuum

from intake, outpatient, inpatient and long term care services. Coordinates

communication between patient|family, internal and external service providers.

Communicates effectively with and works as an integral member of the

multidisciplinary care delivery team

* Demonstrates an understanding of family centered care, patient's medical

diagnoses, prognosis, progress and implications for family support, education

and goal directed treatments and therapies

* Provides triage for families seeking services for improved management and

understanding of chronic illness to determine service needs. Reviews and

organizes intake materials and consults with internal leaders and team members

as needed

* Demonstrates an understanding of the health care system, managed care and

resource utilization

* Ensures scheduling of evaluations and treatment for prospective patients

* Supporting navigation to inpatient acute rehabilitation and long term care by

working with key internal and external stakeholders and multidisciplinary teams

and departments

* Obtains, manages and documents information related to all patient cases

* In alignment with CSH mission, values, purpose statement and Strategic

Initiatives, advocates for the needs of patients|families across the medical, social

and financial continuums

* Provides necessary documentation to external agencies and entities as needed
