WORK ASSIGNMENT DETAILS

Waukesha County's Department of Health and Human Services is seeking a Children's Long-Term Support (CLTS) Service Coordinator to serve in a specialized capacity as a Provider Relations Specialist. This position builds upon the foundational knowledge and skills of a CLTS Support and Service Coordinator and plays a key role in strengthening our provider network. The CLTS Provider Relations Specialist serves as the primary conduit for communication and collaboration between Medicaid-authorized CLTS service providers and the Department.

Key responsibilities include:

  • Relationship Management: Maintain positive, professional working relationships with all CLTS providers and act as their main point of contact for program questions, support needs, and system navigation.
  • Provider Development & Recruitment: Assist in identifying and engaging potential new providers to expand service options for families. Conduct initial outreach meetings and provide clear guidance regarding CLTS Waiver Program expectations, service standards, and compliance requirements.

  • Program Education & Technical Assistance: Offer ongoing education, consultation, and feedback to contracted agencies and individual providers. Support providers in understanding Medicaid mandates, program operations, documentation standards, and case management expectations.

  • Compliance & Quality Oversight: Monitor the CLTS Provider Registry system, help ensure provider compliance with Medicaid regulations, and support the Department's efforts to maintain high-quality, accountable service delivery.

  • Community Engagement: Assist in planning, coordinating, and facilitating large community provider meetings, trainings, and informational sessions as needed to support communication, consistency, and system improvement.

This specialized role supports the broader goals of the CLTS program by enhancing provider capacity, strengthening system partnerships, and ensuring high-quality, compliant services for children and families.

The 2025 salary range for Senior Care Manager is: $29.44 - $40.88.

Senior Care Manager Minimum Training & Experience Requirements
1. A bachelor's degree from a recognized college or university in sociology, social work, psychology, nursing, criminal justice, or a closely related field.
2. Two years of responsible work experience in social work, counseling, nursing, criminal justice, or a closely related field.
3. A master's degree in social work, nursing, psychology, criminal justice, or a closely related field may substitute for one year of the work experience.
4. Social work certification may substitute for one year of work experience.
5. Valid driver's license.

The department may choose to underfill this position as a Care Manager.

The 2025 salary range for Care Manager is: $26.69 - $35.31.

Care Manager Minimum Training & Experience Requirements:

1) A bachelor's degree from a recognized college or university in sociology, social work, psychology, nursing, criminal justice, or a closely related field.

2) Valid driver's license.

3) One year of related work experience in social work, counseling, nursing, criminal justice, or a closely related field.
