Optum Infusion Pharmacy has an immediate opening for a friendly, patient focused and detail-oriented Patient Care Coordinator to join our team.

The primary role of the Patient Care Coordinator on the National Start of Care Nursing Team is to provide nursing coordination for infusion services for all Optum Infusion Pharmacy patients, coordinating the daily activities of referral, intake, staff/agency assignment and patient management. The Patient Care Coordinator works cooperatively with all members of the care team to support the vision and mission of the organization and deliver excellent customer service.

Position Schedule: Monday through Friday with 8-hour shifts between the hours 8:00am-6:00pm PST.

Primary Responsibilities:

  • Receives and executes the initial patient intake process to assure that patient verification and ongoing scheduling of nursing services occur in a timely manner
  • Assures timely communication with other company personnel, and outside providers, especially Optum nursing and other agencies, to assure the quality and continuity of patient care
  • Answer telephone calls promptly and courteously
  • Ability to establish and maintain positive clinical rapport with all patients and their families, representing Optum Infusion Pharmacy including working directly with patients and their families, maintaining contact as per company policy and protocol
  • Communicates coordination of care with alternate providers (AIS, hospice) and documents communication
  • Completes all required patient care documentation and reports accurately and in a timely manner, according to internal and external regulatory requirements
  • Tracks schedule of visits
  • Collaborative relationships involving agencies delivering services on behalf of/or in conjunction with Optum Infusion Pharmacy, including nursing agencies in a contracted relationship, patient, families, significant caregivers, and other healthcare team members
  • Participates in staff meetings with nursing and pharmacy
  • Maintain all mandatory education required upon hire and annually per policy
  • Always represents self in a professional and ethical manner, while representing Optum Infusion Pharmacy, complying with all applicable company policies and procedures
  • Perform other related duties and assignments as required

Required Qualifications:

  • 6+ months of customer service or healthcare related experience
  • Intermediate level of computer proficiency including Microsoft Office Word, Excel and Outlook and multiple web applications
  • Ability to work remotely out of Pacific/Arizona time zone Monday through Friday between the hours 8:00am-6:00pm PST
  • Must be 18 years of age OR older

Preferred Qualifications:

  • 1+ years of experience working in a front office medical position performing duties such as scheduling appointments, checking patients in/out and maintaining medical records
  • Knowledge of EMR software

  • Ability to work independently and as a team, and maintain good judgment and accountability
  • Ability to multi-task and prioritize tasks to meet all deadlines
  • Ability to work well under pressure in a fast-paced environment
  • Demonstrated ability to work well with health care providers
  • Strong organizational and time management skills
  • Excellent verbal and written communication skills; ability to speak clearly and concisely, conveying information in a manner that others can understand, as well as ability to understand and interpret information from others

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The hourly pay for this role will range from $17.74 to $31.63 per hour based on full-time employment. We comply with all minimum wage laws as applicable.
