Job Summary

The CN identifies and implements nursing interventions and evaluates results for a wide variety of patient populations, directs and acts as resource to other members of the nursing team as assigned and promotes compliance and support for policies. The primary responsibilities for this position consist of providing total, comprehensive, continuous and patient centered care which includes: assessing, documenting, planning, implementing, teaching, developing, and evaluating a comprehensive nursing plan of care; delivering direct patient care; ensuring patient satisfaction; training and delegating duties; and adhering to University policies and procedures as well as federal, state and local regulations.

Autonomous nursing care is the ability of a nurse to assess and provide nursing actions as appropriate for patient care populations based on competence, professional expertise, and knowledge. The nurse is expected to practice autonomously consistent with ANA Standards of Practice and Professional Performance. Independent judgment is expected to be exercised within the context of interdisciplinary and multidisciplinary approaches to patient care.

Provides direct patient care to a diversified patient population. Applies knowledge, experience and judgment to determine the importance of a situation, set priorities, and use abstract thought to evaluate clinical situations. Participates in patient and staff education. Demonstrates leadership skills. Demonstrates accountability for own practices as defined by the Nurse Practice Act.

Minimum Qualifications - For full consideration, applicants are encouraged to upload license and/or certification if required of the position

  • Valid California Registered Nurse License without restriction
  • American Heart Association (AHA) or American Red Cross Basic Life Support (BLS), or UC Davis Health CPR
  • Graduation from an accredited school of nursing
  • Minimum of three (3) years of recent RN experience.
  • Recent RN experience in an ambulatory setting, case management of chronic kidney disease, including experience providing telephone triage to adult patients to manage symptoms.
  • Experience and advanced understanding of the medical management of Nephrology patient population with chronic kidney failure.
  • Experience providing patient education and case management of patients with chronic kidney failure.
  • Experience providing ongoing care coordination for patients through telephone triage, EMR messages, obtaining prior authorizations for medications, refilling medications, support patient with appointment scheduling, physician communication and symptom assessment to avoid ED visits.
  • Experience with indwelling lines such as PICC lines, urinary catheters, accessing ports, and central line care.
  • Experience caring for patients receiving medical management of renal conditions and treatments such as Chronic Kidney Disease, Glomerulonephritis, Kidney Stones, Polycystic Kidney Disease, Renal Failure, Pyelonephritis, Nephrotic Syndrome, Hemodialysis, and Peritoneal Dialysis.

Department Overview

This role is an integral part of the Nephrology and Cardiology, supporting clinical operations, patient care, and coordination across multidisciplinary teams to ensure high-quality, seamless care for all patients.
Together, the Nephrology and Cardiology Clinics combine clinical expertise, research innovation, and patient-focused care to improve kidney health and cardiovascular for the communities we serve.
The Nephrology Clinic delivers specialized care for patients with chronic kidney disease, hypertension, complex kidney disorders, and end-stage renal disease. Our nephrologists, nurse practitioners, and support staff emphasize early detection, prevention, and effective management of kidney disease through coordinated, patient-centered care.
The Cardiology Clinic offers a full range of services, including preventive cardiology, interventional cardiology, electrophysiology, heart failure and transplant, structural heart disease, and advanced cardiac imaging. Our team of experienced cardiologists, nurse practitioners, and support staff focuses on personalized, evidence-based care to optimize heart health and improve patient outcomes.

Department Specific Job Scope

This role will be primarily located at HAS (Health Administrative Service) Building in Rancho Cordova and Nephrology clinic support at ACC Internal Medicine Specialty and/ or other UCDH on-site with UCDH personnel and individual patients.

The position will have responsibilities to the chronic kidney disease patient population and cross coverage of the heart failure patient population with support of Cardiology clinical clinicians.
The Clinical Nurse II will support the clinical needs of patients related to the management of general care and or disease-specific care needs, in collaboration with interdisciplinary teams to address patients' questions or concerns regarding disease management, acute decline, hospitalization, or after-discharge needs. The RN will collaborate with interdisciplinary teams to provide holistic care, facilitating the management and communication of medication orders, diagnostic tests, and treatments, ensuring the timely and accurate implementation of physician orders.
* Assess patient needs and develop individualized care plans with the provider, addressing both immediate and long-term needs in collaboration with the case manager.
* Through co-management with CNS and CMs, the CNII will play a pivotal role in addressing patients' immediate needs by acting as a liaison between patients and providers, ensuring timely interventions (such as medication, labs, imaging, access to care, and DME orders). This proactive approach will help avoid unnecessary appointments with long wait times, which often lead to ED visits and unnecessary hospitalizations.
* Provide ongoing support, education, and guidance to patients and their families, focusing on the preservation of kidney function in partnership with the CNS.
* Consistently monitor and manage dialysis treatments, including timely outreach to dialysis centers as directed by providers. Support the administration of medications, blood products, fluids, and the management of vascular access devices as needed.
* Collaborate with physicians and other healthcare professionals to develop and implement comprehensive treatment plans, ensuring coordinated and efficient care.
* Coordinate overall care, from pre-transplant evaluation to readiness for transplant.
Through nursing assessment, including general or disease-specific care management and education, the RN advises and assists patients with appropriate nursing interventions, including symptom and medication management, collaboration with interdisciplinary teams to provide holistic care including facilitation of management and communication of medication orders, diagnostic tests, and treatments. Ensures timely and accurate implementation of physician orders.
