Overview
Helio Health is on a mission to treat and promote recovery from the effects of substance use, mental health disorders, and other behavioral healthcare issues. In order to support the mission, Helio Health has an exciting opportunity for a Wellness Coordinator (LPN) to lead and develop our Residential programs.
In addition to Helio Health's comprehensive benefits package, the Wellness Coordinator is eligible for a Sign-On incentive of $4,000!
Pay Range: $26.00-$28.00/hr.
Responsibilities
Responsible for facilitating wellness initiatives, education and training for program participants and staff.
- Responsible for adherence to appointments, medications prescribed, collaboration with healthcare providers, and relevant support services.
- Responsible for the operations of program medication rooms and audits of medication oversight in accordance with regulations and best practices.
- In conjunction with program staff:
- Provide instruction/guidance/support/training and act as a resource to the Medication Specialist and Resident Specialists of various residential program sites relative to Med Room operations, adherence to regulations/policies, and relevant health topics.
- As needed, assist and model for staff coordination between prescribing physicians, their designees, managed care companies, labs and pharmacies to ensure all requirements for filling prescriptions are to ensure timely supply of medication fills/refills.
- Obtain and maintain all documentation necessary to meet regulations and adhere to the Agency's Medication Policies and Procedures.
- Oversee medication room procedures and organization, reporting any discrepancies or problems to the Program Supervisor and/or their designee.
- Assist, and at times, accompany residents to appointments to ensure communication and coordination with healthcare providers.
- Ensure accurate fulfillment of Physician orders as able relative to program capability and regulatory adherence.
- Report errors and incidents in accordance to Agency Incident Management Policies and Procedures.
- Assist with the t raining of new staff, and the re-training of staff in the Medication policies and procedures.
- Explores changing trends with medications, ability to fill medications, coordination with Physicians, managed care and pharmacies.
- Identify staff training needs related to resident health and facilitate training sessions.
- Identify health education needs for residents and facilitate routine educational sessions to widen resident's understanding of health conditions and wellness opportunities.
- Explores opportunities to advance4 the effectiveness and efficiency of the medication program.
- Works to solve ongoing problems at a systemic or community level to ensure quality services to the residents.
- Other duties as assigned.
Qualifications
- Valid and active NYS Licensed Practical Nurse
- Experience working with individuals with mental health, substances abuse or co-occurring disorders preferred.
- Must possess a valid NYS Driver's License
- Ability to relate effectively with staff, residents and the professional community.
- Ability to perform duties independently
- Effective oral and written communication skills.
