Expand your experience as a Public Health Nurse with a leadership role in the administration of our Home and Community-Based Waiver care management program!

This unique Senior Public Health Nurse position is part of the Department of Health Services team, but housed with the Department of Human Services, Adult and Aging Division, in the Home and Community Based Waiver (HCBA) program. This program provides social and health care management to children and adults with complex medical needs in our community. The collaborative goal of this program is to provide comprehensive case management services to these individuals in order for them to remain in their homes and live safely in their community. This Senior Public Health Nurse position plays a leadership role in the administration of this care management program by evaluating each client to determine if they meet the program requirements as delineated by the Department of Health Care Services. Additional duties include:

  • Providing education and coordinating plan of care with clients, their caregivers, medical professionals, social workers, and community partners
  • Conducting home visits that include thorough nursing assessments for all HCBA clients to address their physical, cognitive, functional, psycho-emotional, nutritional, and rehabilitative needs
  • Developing plans of treatment for each individual they work with, and guiding the HCBA program supervisor and social workers
  • Representing the County of Sonoma HCBA program to other agencies, and communicating with state and community partners on program needs
  • Providing training to new staff for the HCBA program, as well as technical updates and updated program dialogue regarding the HCBA program
