Job Overview: In this role, you will coordinate performance improvement activities according to the Trinitas Hospital Performance Improvement Plan. Qualifications: Required:
- Bachelor's degree in healthcare (Nursing, Pharmacy)
- In lieu of a bachelors in healthcare, a Licensed Practical Nurse (LPN) with related hospital experience will be considered.
- 3 years bedside work experience
Preferred:
- Registered Nurse (RN) license
- 2+ years in quality management/performance improvement experience
- Experience in leading teams through process improvement and change initiatives
- Proficient in PowerPoint, Word, internet literature searches, use of audiovisual equipment and development of multimedia educational program development
Scheduling Requirements:
- Shift working, 8:00am-4:00pm Monday - Friday

Essential Functions:
- Utilizes principles of continuous quality improvement in all work situations to assess measure and improve organizational and department functions
- Creates teaching material to support Organizational needs
- Assist in the communication and coordination of clinical services initiatives
- Assist in EMR chart reviews with medical assistants to identify potential errors in charting
- Collaborates with other departments to create and maintain safe and efficient practice workflows
- Creates teaching material to support Organizational needs
- Plays a critical role in on-board training of new clinical staff
- Will perform clinical skills competencies for all clinical staff
- Maintain electronic medical record EMR proficiency
- Conduct Occupation Safety and Health Administration OSHA training for staff
- Coordinates patient safety activities across the organization as assigned, including but not limited to, quality, compliance, and performance audits, surveillance activities, prospective risk assessments and analyses and intensive analyses
- Conducts RCAs to investigate occurrences, identifying proximate causes, developing root causes and drafting corrective action plans for discussion with key stakeholders
- Serves as a change agent develops, coaches and facilitates complex process optimization projects as assigned, in order to achieve measurable progress to achieve strategic priorities
- Provides thought leadership and recommendations for improving workflows, team structures and system configurations
- Rounds with staff and leaders in a continual pursuit of patient safety improvement opportunities
- Establishes and maintains the objectives and strategy for quality and patient safety professionals and team members in order to effectively contribute to executing the overarching CSH strategic plan
- Participates in reporting and communication of quality improvement and patient safety initiatives, quality and patient safety awareness and safety culture survey administration
- Coordinates tracking and reporting of clinical outcomes of assigned clinical indicators and quality improvement initiatives e,g,National Database for Nursing Quality Indicators and other registries and databases
- Facilitates communications between nursing staff, leadership, and shared governance councils to explore solutions and select best alternatives for evidence based clinical practices
- Serves as a member of the Nursing Executive Committee providing strategic input on initiatives related to nursing excellence, quality, safety and professional nursing practice
- Works with organization stakeholders to define existing and new processes and recommend solutions and improvements as they affect accreditation and or regulatory requirements
- Serves as a resource for leaders and staff on patient safety and quality improvement activities, education and use of patient safety and high reliability tools
- Directs the implementation of programs by evaluating the quality of services provided through gap analyses process improvement activities, event reporting, patient safety standards, etc,, review of reports e,g, Press Ganey, Joint Commission surveys, Sentinel Event reports, satisfaction surveys, etc, and risk assessments
- Fosters the use of systematic evaluative research data with regard to process and performance improvement to facilitate implementation of an evidence based practice across the organization
- Identifies and assists staff across the organization in finding opportunities to improve patient care workflow, work processes and quality, and then works collaboratively to implement and monitor improvements
- Participates and coordinates in High Reliability activities as directed, including but not limited to overseeing the Safety Coach program and mentoring Safety Coaches in their role, the development and implementation of unit-based safety huddles, error prevention tool refresh, among others
- Oversees the planning and implementation of internal quality fairs and submission of abstracts to external quality and safety conferences
- Demonstrates effective leadership skills including systems thinking, critical thinking, problem-solving, goal setting, decision-making, delegation, strategic planning, project management, coaching and mentoring
- Evaluates own practice in relation to professional practice standards, relevant statutes and regulations, maintaining continuing professional nursing competence and teaching and learning effectiveness
