Description

The Social Worker (SW), communicates and collaborates closely with physicians and other members of the health care team to provide consistent quality care for patients. Working with a high degree of autonomy, the SW accepts responsibility and accountability for assigned patients to plan implement and evaluate the care for patients. The focus of the SW will be on social determinants of health and addressing issues identified.

This position requires excellent teaching and/or communication skills (individual/group) which will be essential in the process with patients. In collaboration with the physician, RN Care Managers, other care providers, the SW evaluates, plans, and revises care as needed. Performance of duties must sincerely and positively reflect consideration, concern, and inspire confidence when interacting with patients, visitors, physicians or fellow employees.

The primary goal of the SW is to effectively manage patients at high risk for healthcare complications and improve their quality of life. Through a collaborative process of assessment, planning, facilitation, care coordination, evaluation and advocating for options and services to meet a patient's needs, the SW coordinates with providers and RN Care Managers. Coordinating communication with nurses and providers is a high priority.

Required Qualification:



  • Bachelor's Degree in Social Work, Psychology or human services field


Preferred Qualification:



  • 3 years of Social Work experience. Strong professional level of knowledge and comprehensive clinical assessment skills.
