Job Summary

The Case Management Supervisor position on the Horizon House Housing Team exists to provide day-to-day clinical guidance to direct support staff delivering case management services to individuals and families experiencing homelessness and those who are housed within Horizon House programs. This role is responsible for administrative and supervisory functions, supporting staff through crises, and ensuring high-quality data and documentation. Services are delivered using a Strengths-Based, Harm Reduction approach within a Housing First model, with housing stability as a primary program goal. The Case Management Supervisor also supports staff training and development and ensures the team has the tools and resources needed to meet program standards. This position reports directly to the Program Manager (HTR).

Example Duties and Activities

  • Provide day-to-day clinical supervision to Intensive Case Managers within the housing program, offering guidance and expertise related to housing stability, service delivery, and best practices.

  • Provide oversight and coordination of client treatment goals and service plans and assist staff in navigating complex cases and crises.

  • Manage referrals from the Coordinated Entry system, assign clients to Intensive Case Managers, and track referrals in accordance with case conferencing guidelines.

  • Ensure Intensive Case Managers maintain timely, accurate, and complete documentation, including client demographics, service records, and case notes.

  • Ensure all documentation of client encounters and services complies with Horizon House standards and Indianapolis Continuum of Care requirements and is accurately entered into case management system.

  • Assist in maintaining positive and productive relationships with landlords, property managers, the Indianapolis Housing Agency (IHA), and community partners.

  • Participate in and support multidisciplinary and community-based case conferencing to advance client progress toward housing and service goals.

  • Attend and actively participate in Housing Team meetings and other Horizon House meetings as assigned; represent Horizon House at community meetings and events as requested.

  • Complete staff reviews, evaluations, coaching, and corrective actions in accordance with Horizon House policies and procedures.

  • Supervise practicum students as assigned.

  • Model and enforce ethical and legal practice, including adherence to confidentiality requirements, mandated reporting laws, and agency protocols.

  • Attend required agency trainings and participate in position-specific and community-based professional development activities.

  • Assist with onboarding, training, and ongoing development of staff and, as appropriate, community partners.

  • Provide education and guidance to staff related to housing processes, documentation standards, and service delivery expectations.

  • Collaborate with the Housing Program Manager to support new housing initiatives, including developing, documenting, and implementing program processes and procedures.

  • Perform additional duties as assigned to support the mission, vision, and goals of Horizon House.

Required Competencies

  • Degree and Credential Requirements - Master's degree in Social Work (MSW) or related field required.

  • Industry Experience - Prior experience providing services to individuals experiencing homelessness or low-income populations preferred. Experience may include employment, internships, volunteer work, or academic study. Working knowledge of insurance systems and services related to in-home care, hospital stays, and rehabilitation services.

  • Program Management- Demonstrated knowledge of Housing First principles, harm reduction practices, and strengths-based service delivery models, with the ability to apply them in daily work. Working knowledge of chronic medical conditions, mental health diagnoses, substance use disorders, and evidence-based treatment approaches. Working knowledge of insurance systems and services related to in-home care, hospital stays, and rehabilitation services.

  • People Leadership- Supervisory experience preferred, with the ability to coach, support, and develop staff in a clinical and housing-focused environment. Ability to work collaboratively with internal teams, outreach staff, community partners, and external stakeholders.

  • Strategic & Business Acumen - Understands the intersection of social services and operational efficiency; able to balance mission with measurable outcomes.

  • Building Collaborative Relationships - Develops, maintains, and strengthens partnerships at all levels with others inside or outside the organization who can provide information, assistance, and support. Is able to receive as well as provide feedback to improve performance or processes; builds and maintains customer satisfaction.

  • Information Monitoring/Reporting - Collects, analyzes, and applies program data to inform strategies and improve services. Experience in data reporting systems is essential.

  • Communication Skills - Articulates thoughts and expresses ideas effectively using oral, written, visual, and non-verbal communication skills, as well as listening skills to comprehend customers' needs. Delivers information in person, in writing, and in a digital world.

  • Organizational Aptitude - Plans and attends to details and pursues quality in accomplishing multiple tasks in an organized and timely fashion. Exhibits careful attention to detail and follow-through.

  • Technical Knowledge - Proficiency in Microsoft Office and Google Workspace applications, with the ability to learn and utilize electronic databases, HMIS, and case management systems.

Other Requirements

  • Background Screening - All candidates must pass a federal background check.

  • Driving- Must have a valid driver's license, up-to-date auto insurance, and a good driving record.

  • Ability to work a flexible schedule beyond normal business hours.

  • Is willing to travel to participants' homes, workplaces, community centers, and other venues at times that are convenient to individuals being served and be able to transport program participants as needed.
