Case Manager Agape (Full Time)

The Agape Center serves individuals living and thriving with HIV/AIDS. Case managers work closely with clients to assess needs, develop a case plan, and support them in meeting basic needs and achieving life goals. Care coordination with the Infectious Disease provider is essential. Services are delivered at the agency, as well as in the home and community. We have a small team that works closely together. If you are a positive, high-energy person, you are perfect for us!

Education/Licensing Requirements:

Associate's Degree preferred. Experience can be substituted. Must have demonstrated case management skills and experience; emergency intervention methods, family dynamics, health, mental health, and substance abuse systems; and knowledge of community resources. Knowledge of HV/AIDS is necessary. If no prior work with this specific issue, then one must show closely related and commit training. Spanish-speaking is essential. Valid driver's license, auto insurance, and reliable vehicle required.
