Assertive Community Treatment (ACT) Team


Hourly Pay Range: $27.13 - $44.88



SIGN ON BONUS $7,500

Job Summary: Registered Nurses for the Assertive Community Treatment (ACT) Team direct and provide comprehensive nursing care for consumers through medical assessments, psychiatric assessments, medication injections, health education, recommendations, and other services.



Essential Responsibilities:



  • Communicate with team psychiatrist about consumer prescriptions. Monitor, dispense, and document client medications. Act as liaison between psychiatrist and pharmacy. Maintain storage and documentation of onsite client prescriptions.
  • Conduct individual medical and nursing assessments, including at time of intake. Routine assessments thereafter at least every six months. Work with consumer and team to establish rehabilitative goals to further the consumer's medical needs and physical health.
  • Provide direct case management services to a small caseload to help consumers successfully live in the community. Brokerage of services outside of ACT team, regular collateral contacts of providers, and others involved in the consumers' lives.
  • Provide crisis response services to individuals who are experiencing a psychiatric crisis. Interface with county operated crisis services, hospitals, and others as needed to be included in a rotation of after hours and on call services.
  • Conduct individual and group sessions to give consumers' tools to manage their physical health and medical co-morbidities.
  • Complete clinically sound and timely documentation of all services rendered on behalf of consumers in the Electronic Medical Record. Maintain a productivity level of 75% or greater.
  • Perform all other duties as assigned.



Minimum Requirements:



  • Valid United States Driver's License and full access to a reliable, insured vehicle, both able to be maintained throughout employment.
  • Associate degree or higher in Nursing.
  • Possession of valid Wisconsin Registered Nurse licensure without restriction at the time of application, able to be maintained throughout employment.
  • At least one (1) year of experience of psychiatric nursing in a community setting.
  • Advanced knowledge of all related computer and software applications, such as word processing and spreadsheets.
  • Advanced knowledge of electronic health records.
  • Able to lift/carry and push/pull up to 30lbs.
  • Willing to drive ~50% of the time in this role.
  • Willing to be part of a rotation for after hours and weekend on-call services.



Preferred Qualifications:



  • Bachelor's degree or higher in Nursing.
  • Bi-lingual in Spanish.



NOTE: This original application is open to permanent residents of the United States. Appointee must establish residency in the state of Wisconsin within six months of start date and maintain such residency during incumbency. In accordance with Section 11, Rule 3 of the Civil Service Rules a non-competitive examination will be held for this position. The non-competitive examination will consist of criteria deemed appropriate by the Director of Human Resources.
