Position Summary This position requires the full understanding and active participation in fulfilling the mission of Saddleback Medical Center. The employee shall support Saddleback Medical Center's strategic plan and participate in and advocate performance improvement/patient safety activities. As a member of the Interdisciplinary Hospice and Palliative Team, the Bereavement Coordinator is responsible for the Hospice and palliative bereavement program, and oversees the counseling of hospice and palliative patients, caregivers and families and bereavement services provided. Counseling includes emotional, psychosocial and spiritual support and services provided before and after the death of the patient to assist with issues related to grief, loss and adjustment. Essential Functions and Responsibilities of the Job 1. Develops and coordinates an organized bereavement program for familiesandresidents of nursing facilities. 2. Oversees that the interdisciplinary team makes an assessment of survivor risk factors at thetime of the patient's admission and continually monitored and develops an appropriate plan for follow-up services needed. 3. Works with the Volunteer Coordinator to provide education and training for the volunteers who provide services for the bereavement program. 4. Ensures that community bereavement support groups are provided weekly. 5. Supervises and ensures complete and current documentation of family assessments, plans andinterventions 6. Provides bereavement education for staff and the community upon request. 7. Be at work and be on time 8. Follow company policies, procedures and directives 9. Prioritize and multitask *Placement in the pay range is based on multiple factors including, but not limited to, relevant years of experience and qualifications. In addition to base pay, there may be additional compensation available for this role, including but not limited to, shift differentials, extra shift incentives, and bonus opportunities.
