Physical Therapist (PT) - PACE Program About the Role: We are seeking a passionate and experienced Physical Therapist (PT) to join a multidisciplinary care team at a reputable PACE (Program of All-Inclusive Care for the Elderly) facility in the Louisville Metro area. Evaluate and assess participants' physical function, mobility, gait, balance, and rehabilitation needs in both home and center environments.
Develop, implement, and modify individualized physical therapy care plans in collaboration with the interdisciplinary team.
Provide therapeutic interventions to help participants improve or maintain mobility, reduce fall risk, and enhance physical independence.
Instruct participants, caregivers, and family members in therapeutic exercises, mobility techniques, and use of assistive devices.
Document assessments, treatments, and progress in accordance with PACE program standards and regulatory guidelines.
Participate in care planning meetings and coordinate closely with RNs, PCPs, OTs, social workers, and other team members.
Qualifications:
Licensed Physical Therapist (PT) in the State of Indiana (or eligible for licensure).
Minimum 1 year of experience in geriatrics, home health, SNF, outpatient, or PACE preferred.
Excellent interpersonal and communication skills; able to thrive in an interdisciplinary care model.
Proficient in EMR systems and documentation best practices.
Valid driver's license and reliable transportation (home visits may be required).
