CAREGIVER SUPPORT SPECIALIST

Caregiver Support Specialist
Washington County, located in West Bend, Wisconsin, has an excellent opportunity for a Caregiver Support Specialist within our ADRC Department. Under the supervision of the Director, ADRC, the Caregiver Support Specialist provides information and assistance to people calling for caregiver supports and options for respite care. The position provides case management for clients receiving funding through caregiver support programs. This position will also provide support to the Caregiver Coalition, support groups and educational programs for caregivers.

Duties/Responsibilities:
The following duties are normal for this position. These are not to be construed as exclusive or all inclusive. Other duties may be required and assigned.
*Completes intake and caregiver assessments for new program participants and annually.
*Conduct home visits with program participants as needed.
*Provide options for agency support and referrals to agency programs
*Create authorizations with agencies providing services to participants
*Communicates with agencies providing services regarding participant programming, participant funding balances grievances and other communication as needed
*Approves and submits invoices for payment, tracks spending of participants as well as monitors yearly budget.
*Data entry in client tracking database.
*Attends and participates in mandatory meetings and any other training as directed.
*Work with GWAAR as needed regarding annual reporting, bi-monthly Statewide Caregiver Coordinator virtual meetings.
*Work with Accounting Assistant to reconcile accounts and GWAAR reports.

*Performs other duties as assigned, including responding to an emergency event.
*Assist with Caregiver Coalition events and provide educational programming, as needed.

Works under the routine supervision of the Director.
Supervision Exercised:
None.
Decision Making:
Decisions concerning prioritizing work, assisting in determining eligibility, providing information as needed.
Interaction:
There is interaction with consumers and their families. ADRC staff, other county departments as well as the general public.
Essential Knowledge and Abilities:
Ability to work independently. Ability to communicate with older adults and adults with disabilities and good listening skills. Ability to comprehend government benefits and programs regulations. Able to fulfill physical demands of job, sensory demands (seeing and hearing) and cognitive demands (conceptualization, concentration and memorization).
Training and Experience:
Bachelor's degree in a social work or a human services related field is preferred. Minimum of 2 years of experience in the Human Service field required. Experience providing support to caregivers, older adults or those with disabilities. A valid Wisconsin driver's license, available automobile, and automobile insurance are required.
