Position Title: LTSS Care Navigator

Work Location: San Antonio, TX Remote

Assignment Duration: 3 months

Work Arrangement: 8am-5pm CST

Optional OT is available, not mandatory

Position Summary:

This role is interesting because you get to connect with our members or parents of children with special health care needs and challenges and assist them.

Team is highly engaged and supportive of each other, always willing to help each other.

Background & Context:

Fast-paced role supporting members with special health care needs at The Organization.

Key Responsibilities:




  • Connect with members or parents of children with special health care needs and challenges



  • Assist members through telephonic work



  • Gather information via probing to support decision-making




Qualification & Experience:




  • Required Education: HS Diploma



  • Preferred Education: Bachelor



  • Must haves:




    • Excellent customer service skills



    • Strong computer skills



    • Attention to detail



    • Telephonic work experience



    • Reliable internet connection





  • Nice to haves:




    • Bilingual





  • Disqualifiers:




    • No experience working with Medicaid and special health care needs





  • Performance indicators:




    • Good work ethic



    • Time management



    • Organization



    • Productivity log participation (tasks are very fluid)





  • Best vs. average indicators:




    • Quick learner



    • Engaged



    • Asks valid questions



    • Cares about quality of work



    • Takes initiative



    • Cares for members' needs





  • Top 3 must-have hard skills (stack-ranked):




    1. Computer skills - high level of experience, important (Microsoft Office)



    2. Communication skills - high level of experience, majority telephonic



    3. Decision-making skills - able to gather information via probing
