Wages & Benefits

$22.00 - $24.00 per hour; Generous Paid Time Off (PTO) policies, plus eight paid holidays and two floating holidays; competitive health, dental, and vision insurance. There is employer paid basic life insurance with the option for additional coverage paid by the employee. The employer pays the premiums for Accidental Death and Dismemberment (AD&D) and long-term disability insurance. H4Y sponsors a retirement plan with a 3% employer match and immediate vesting.

Schedule

This position will be fully in-person and includes work during normal business hours as well as evening and/or weekend hours.

Job Summary

HOPE 4 Youth is hiring an Outreach Specialist to provide quality case management and assistance to youth experiencing homelessness at our HOPE 4 Youth Center in Anoka. The Outreach Specialist will provide access and referrals to basic needs and supplies, community resources and services, and will work individually with youth to set personal goals, provide encouragement, and give them the support they need on their path towards independence and self-sufficiency.

Responsibilities



  • Providing case management to youth in alignment with their goals toward sustainable housing.
  • Completing intakes and assessments for new youth participants.
  • Providing youth with referrals to community resources and services.
  • Documenting services, case notes, and case management hours in ClientTrack.
  • Maintaining a safe and supportive environment for all youth.
  • Providing guidance and support to volunteers and interns to ensure policies, procedures, and protocols are followed and participant needs are met.
  • Maintaining safety policies and procedures within the HOPE 4 Youth Center.
  • Assisting with creating and maintaining outreach program data tracking and metrics.
  • Developing partnerships with schools, organizations, and local businesses to build positive outreach relationships.
  • Meeting youth in the community when requested.



Who You Are



  • You hold a high school diploma or GED.
  • You bring at least three years of professional or lived experience.
  • It is preferred, but not required, if you have experience providing case management to at-risk or homeless youth ages 16-24.
  • You have previous work experience in outreach.
  • You demonstrate the ability to work effectively in a diverse, mission-driven organization committed to diversity, equity, and inclusion across race, ethnicity, gender identity, sexual orientation, socio-economic status, nationality, and religion.
  • You value volunteers as an integral part of organizational culture and have the ability to manage, supervise, work alongside, and build strong relationships with volunteers.
  • You have the ability to establish and maintain effective relationships with youth, families, and community service providers.
  • You demonstrate a strong understanding of crisis intervention and programming that is trauma-informed, strengths-based, client-centered, and grounded in harm reduction.
  • You are proficient in Microsoft Word and Excel and can produce professional-quality letters, emails, and reports.
  • It is preferred if you have experience working with databases and data.
  • You are fluent in English, with fluency in a second language preferred.
  • You value working in a highly motivated, team-oriented environment.
  • You have a valid driver's license, access to reliable transportation, and the ability to meet H4Y's driving record requirements. Please note that if the license was not issued in the state of Minnesota, you have 60 days after becoming a Minnesota resident in which to apply for your Minnesota driver's license or permit (not applicable for new hires living out of state).
