THE POSITION

The Department of Human Services is looking for a dedicated Patient Care Coordinator to join our vital Utilization Review Program at Norristown State Hospital. DESCRIPTION OF WORK

As a Patient Care Coordinator, you will play a vital role in managing the continuity and quality of inpatient care. Your primary responsibility involves the timely monitoring of admissions and continued stays within the electronic health record (EHR) to ensure that ongoing hospitalization remains medically appropriate. To maintain clinical integrity, you will conduct rigorous quality reviews of active patient records, evaluating the documentation from all treatment team members for both frequency and content quality. All work must align strictly with the hospital's utilization review (UR) plan, as well as federal and state regulations, Medicare standards, and internal policies.

In this role, you are responsible for assigning and certifying appropriate lengths of stay based on medical necessity. You will be expected to generate and distribute EHR feedback to the relevant departments while monitoring any necessary follow-through. For complex or questionable cases regarding admissions or extended stays, you will escalate the matter to the UR Advisor. Beyond daily monitoring, you will take a leadership role by chairing or co-chairing the UR Committee meetings, where you will provide supportive data and recommendations on case disposition.

Furthermore, you will act as a key liaison for insurance and financial compliance. Your work will include coordinating managed Medicare pre-certification reviews and performing concurrent reviews to secure continued stay approvals for billing purposes. Your will also collaborate closely with physicals and social workers to organize the documentation required for Medicaid applications. Finally, you will be responsible for preparing and presenting essential reports for the Performance Improvement Council and the UR Committee to drive institutional excellence.

Work Schedule and Additional Information:

  • Full-time employment, 37.5 hours per week
  • Work hours are 8:00 AM to 4:00 PM, Monday - Friday, with a 30-minute lunch. This may change based on operational needs.
  • Travel as operationally necessary

REQUIRED EXPERIENCE, TRAINING & ELIGIBILITY

QUALIFICATIONS

Minimum Experience and Training Requirements:

  • One year as a Psychiatric Nurse 2 in a state hospital or a Nurse 2 (Commonwealth job title or equivalent Federal Government job title, as determined by the Office of Administration) in a State General Hospital or Restoration Center; or
  • Three years of professional psychiatric nursing or general nursing, including one year at an advanced level in a state hospital, general hospital or Restoration Center; and graduation from an approved school of professional nursing; or
  • Two years of professional social or rehabilitative work, including one year of clinical experience in a supervisory, administrative or consultative capacity in a state hospital, state general hospital or Restoration Center; and a bachelor's degree with major course work in social work, nursing, health care administration, behavioral sciences or related fields; or
  • Any equivalent combination of experience and training.
