The Social Worker is responsible for performing social work assessments and interventions for ambulatory patients in the BWH Department of Neurology, Division of Multiple Sclerosis. They are dedicated to providing trauma-informed, patient-centered clinical care, skilled at managing multiple priorities at once, and engaged in creative, compassionate, and ethical problem-solving. As a member of a multidisciplinary health team, they perform comprehensive assessments and develop treatment plans that address the medical, social, and resource issues that impact patients and their families who are diagnosed with a range of seizure disorders.
Does this position require Patient Care? Yes Essential Functions: - Provide disease education and supportive counseling to patients and families, related to adjustment to diagnosis and living with chronic, progressive illness as well as individual and family decision making related to changes in health status. - Collaborate with interdisciplinary team to evaluate patients' medical or physical condition and to assess client needs. - Mobilize personal, hospital and community resources for identified patients to reduce barriers to engaging in overall health care. - Act as a liaison with a wide range of collateral systems for effective patient care. This includes specific liaisons with Epilepsy Foundation and other community agencies. - Oversee the update of community resource information. - Participate in psychosocial research with patient population. Data collection, quality initiatives, and special projects as requested. - Develop and run groups for patient population on disease specific issues.
