The Nurse Practitioner (NP) Coach serves as a pivotal member of the IKC Clinical Services team, dedicated to elevating the performance of our front-line clinical leaders. This role focuses on coaching and mentoring Nurse Practitioners (NPs) to excel in two core areas: proactively reducing hospital admissions for their patient panels and effectively serving as clinical lead of their care pod.

The NP Coach is responsible for equipping NPs with the skills to lead their teams-comprising Registered Nurses (RNs) and Care Coordinators (CCs)-in successfully managing a full patient roster. This requires a deep clinical understanding of CKD/ESKD, a passion for mentorship, and a strong ability to translate data into actionable coaching strategies that improve patient outcomes and reduce the total cost of care.

Key Responsibilities

1. Coaching for Admission Reduction & Clinical Excellence: * Coach NPs on the early identification of hospital admission drivers and the creation of impactful admission reduction plans. * Provide ongoing mentorship to NPs on managing complex patient panels, with a primary focus on strategies to lower hospital admissions and readmissions. * Support NPs in implementing best practices for managing common co-morbidities such as diabetes, hypertension, and cardiovascular disease to improve patient stability. * Stay current with the latest evidence-based guidelines and clinical research, disseminating this information to NPs to drive superior patient outcomes.

2. Leadership Development & Pod Management: * Guide NPs in developing essential skills as clinical pod leaders, enabling them to effectively lead their pods* Mentor NPs on strategies for managing patient rosters, delegating tasks, and fostering a collaborative and accountable team environment within their pod. * Act as a role model for professional leadership, fostering a culture of continuous improvement and shared decision-making.

3. Quality Improvement & Performance Optimization: * Collaborate with clinical leadership to ensure NPs meet key performance indicators, especially those related to admission reduction and other TCOC initiatives. * Assist NPs in analyzing performance data to identify opportunities for enhancing care quality, improving patient outcomes, and increasing patient satisfaction. * Provide specialized support for initiatives such as Re-Admit Reviews, Optimal Starts (Drive to 65), and EPM huddles.

4. Data Analysis & Reporting: * Track and analyze key performance indicators related to NP practice, pod performance, and patient outcomes, with a focus on admission trends. * Provide regular reports on coaching progress, quality improvement initiatives, and the effectiveness of admission reduction strategies. * Use data to identify trends, pinpoint areas for improvement, and enhance NP effectiveness in managing their patient panels.

Qualifications

  • Master's Degree in Nursing (MSN) or Doctor of Nursing Practice (DNP) required. Active Nurse Practitioner license in the state of practice.

  • Minimum of 3 years of clinical experience as a Nurse Practitioner, with a strong background in CKD and ESKD care and demonstrated longitudinal co-morbid management.

  • Experience in mentoring, coaching, or training other healthcare professionals is highly preferred.

  • Proven experience working within an interdisciplinary team environment.

  • Strong understanding of CKD/ESKD disease processes and management of related co-morbidities.

  • Exceptional communication and interpersonal skills, with a proven ability to build rapport and influence clinical teams.

  • Experience with quality improvement initiatives and data-driven decision-making.

  • A passion for teaching, mentoring, and developing others professionally.

The Salary Range for the role is $98,000.00 - $155,000.00/year.
