MINIMUM QUALIFICATIONS:

Education:
|
Be a graduate of an accredited school of professional nursing
|
Experience:
|
One year experience in a healthcare setting is preferred Maintain a Nursing Licensure by obtaining appropriate number of annual CEU hours
|
Knowledge, Skills & Abilities:
|
Work respectfully with staff, patients, family members and visitors
Properly use/maintain supplies and equipment
Be able to work as a team with other departmental staff
Be able to follow written and oral instructions concerning patient care, and facility up keep.
Must be able to uphold the standards of the MN Department of Health; be familiar with and abide by the federally mandated HIPAA regulations which protect the confidentiality of our patients and employees.
A sincere desire to provide direct services to patients, with a strong emphasis on rehabilitation
Must be able to meet the physical demands of the position (See physical demands requirements)
Must be able to make sound judgments and appropriately prioritize work
Must be able to manage time, be responsible, mature, and flexible

Must be able to work as a team member to maintain consistence and quality of care for all of our patients
Possess good assessment/direct care skills
Possess a working knowledge of IV Therapy
Be fluent in both spoken and written English in order to communicate effectively with patients, families, physicians and co-workers

MAJOR JOB DUTIES AND TASKS: 1. Be responsible for admissions:
Room preparation, welcome, and escort the patient to the room
Introduction to facility, routine
Review and implement admitting orders, enter orders into EPIC
Communicate with physician any concerns or order clarification
Communicate with other departments re: follow-up of physician's
orders
Complete initial nursing assessments, flowsheet and care plans
Provide equipment appropriate to diagnosis, set up
Complete all documentation as required
2. Be responsible for discharges:
Complete discharge or transfer form
Prepare all discharge medications and supplies. Review AVS in EPIC
Double check that all home care needs and transportation have been arranged
Meet with patient and family to review all discharge orders and answer any questions, call in medications to the Pharmacy of choice
Escort patient to vehicle
Complete all documentation as required
3. Provide direct care to patients
Provide ADLs including grooming, bathing, dressing toileting, ambulation, and feeding (includes passing meal trays)
Assess physical and mental status of patient to determine efficacy of treatment plan; includes an accurate head to toe assessment and documentation in EPIC flowsheet. Progress notes written to be diagnosis specific.
Administer medication as ordered by MD/NP team: Oral, IV, PR, inhalers, gtts, topicals.
Provide treatments as ordered by the Physician Team: dressings, PVR, straight cathing, ace wraps, etc. and document the treatment as done.
Communicate with interdisciplinary team as needed

Have a working knowledge of all state, federal, and facility policies
and comply with them.
Complete documentation in chart including nurse's notes, flow sheets,
I&O, BM, VS, O2 sats., and weights
Notify physician and/or nurse practitioner of any patient concerns
Check MARs against new orders
Give report to Nursing Assistants, supervise and work together with nursing assistant to ensure their work is done to meet the MD orders and Patient needs.
Maintain accurate intake and output on patients
Provide a complete and comprehensive report to the on-coming shift
Promote and maintain a positive attitude at all times, professional attitude and interactions with co-workers
Work as charge nurse/in-house coordinator as needed
