Annual Salary of $46,800

Job Summary:

This position is responsible for the overall supervision of Care Companions in assigned geographic areas. The responsibilities include recruiting, onboarding, training, and placing new Care Companions in assigned geographic areas. The responsibilities also include engaging members (clients) in service by providing direct care to increase socialization and connect to community and resources in an assigned geographic area.

The responsibilities include program outreach, friendly visits (telephonic and in-person), light household chores, personal errands such as grocery shopping and transportation. This position is also responsible for documenting direct care visits of members in service, assuring compliance with program regulations, and managing relationships with multiple stakeholders in the community. This position will also be responsible for the provision of direct client services, including LSS Healthy Transitions and similar services, under the supervision of a certified staff member.

Qualifications and Requirements:



  • Bachelor's degree in the field of Human Services or equivalent experience of 5 years is required.
  • Supervision of staff development experience required.
  • Ability to work well with diverse individuals, families, groups and organizations and demonstrate compassion, patience, flexibility and respect for others.
  • Provide regular in-home/telephonic visits to members and provide transportation in the community as needed.
  • Have dependable transportation and be willing to transport members in your vehicle.
  • Computer skills, bookkeeping, computer spreadsheets and word processing skills required.
  • Excellent communication and customer service abilities required.
  • Organizational skills, problem-solving and the ability to work both independently and as a member of a team.
  • A valid driver's license, a clean driving record and background checks are required.
