Knowledge, Skills, and Experience:
- Knowledge of public health principles, practices, health education, and behavioral theories.
- Experience with evidence-based Alzheimer's Disease and Related Dementias (ADRD), chronic disease management, or health education programs.
- Public health program/project management focused on ADRD and chronic disease management.
- Experience developing program goals and objectives.
- Program monitoring and evaluation experience.
- Experience working with partners, coalitions, clinicians, providers, insurers, and employers.
- Grant and contract management experience, including grant writing.
- Ability to provide consultation and technical assistance to partners, including payers and healthcare providers, on evidence-based dementia programs
- Experience conducting trainings, seminars, and group presentations for diverse audiences.
- Ability to interpret regulations and policies.
- Strong skills in evaluating performance, setting priorities, and problem-solving in a positive manner.
Mandatory Qualifications:
- Bachelor's degree in Public Health, Community Health, Health Education, or Nursing.
- Relevant professional experience, or an equivalent combination of education and experience.
Preferred Qualifications:
- Master's degree in public health, health education, nursing, or a related field.
- Experience designing, coordinating, and evaluating ADRD, chronic disease, or health education programs.
- Experience working in community and clinical settings.
- Experience collaborating with health systems, employers, community-based organizations, state-level entities, and key stakeholders.
Duties and Responsibilities:
- Serve as the Health Systems Coordinator for Healthy Brain Virginia (federally funded program).
- Manage healthcare-related strategies and activities under the program.
- Provide day-to-day oversight of healthcare professionals and healthcare settings.
- Develop and implement communication plans to increase ADRD awareness, outreach, screening, referrals, and service availability.
- Develop and promote tools and resources to support healthcare providers.
- Support identification processes, EHR prompts, counseling, and referral workflows.
- Serve as a member of the Virginia Alzheimer's Disease and Related Disorders Commission.
- Oversee contractor performance and progress.
- Position is fully remote and aligned with the current Healthy Brain Virginia team structure.
