Job Overview:

RWJBarnabas Health is looking to add a qualified part-time a Licensed Social Worker to join the Peds Pulmonary/Cystic Fibrosis department in Eatontown NJ. This position provides direct clinical social work services to patients and their families in an interdisciplinary team approach.

Qualifications:

Required:

  • Must have BSW in Social Work
  • Must have at least 2-3 years of experience

Scheduling Requirements:

  • Day Shift, 24 hours/week
  • Part-Time Schedule
    • Tuesday, Wednesday, Thursday (8 AM - 4:30 PM)

Essential Functions:

  • Function under the direction of the CF Center Physicians and Program Coordinators
  • Conduct annual screening for anxiety/depression for Adult and Pediatric patients with Cystic Fibrosis
  • Screen for Food Insecurity and provide resources as needed
  • Provide referrals for psychosocial counseling as needed
  • Record all pertinent psychosocial notes in patient records
  • Be a functioning multidisciplinary team member
  • Regularly attend and contribute to team huddles (8am Tues-both programs/8:30am Thurs-Adult Program)
  • Participate in ongoing continuing education i,e, North American CF Conference (NACFC) and Mid-Atlantic Conference, RD/SW Consortium
  • Facilitate virtual CF Parent/Patient Advisory Committee Meetings
  • Facilitate Quarterly Newsletter
  • Participate in Quality Improvement Initiatives
  • Execute requests for FMLA, Disability, 504 Plans, transportation to clinic visits
  • Track enrollment in patient assistance programs
  • Obtain Advance Directives for patients >18 years
  • Write letters for patients as requested by physicians
  • Maintain email database for Adult and Pediatric CF Programs
  • Data entry in Cystic Fibrosis Foundation Registry
  • Provide contact information for foundations who offer financial help for fertility treatment, testing not covered by insurance, scholarship money for college
